妊娠中晚期体重增加对妊娠期糖尿病孕妇糖脂代谢及胰岛素抵抗的影响
Effects of Weight Gain in the Mid and Late Trimester of Pregnancy on Glycolipid Metabolism and Insulin Resistance in Gravidas with Gestational diabetes Mellitus

Abstract
Objective:To investigate the effects of weight gain in the mid and late trimester of pregnancy on glycolipid metabolism and insulin resistance in gravida with gestational diabetes mellitus(GDM).Method:A total of 112 gravidas with GDM who were admitted to Yichun Maternal and Child Health Hospital from January 2021 to January 2024 were selected as GDM group,and 50 normal pregnancy gravidas who were prenatal screened in our hospital during the same period were selected as the control group.Glycolipid metabolism indexes[fasting blood glucose(FPG),glycated hemoglobin(HbA1c),total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholestero(HDL-C),low-density lipoprotein cholesterol(LDL-C)]and insulin resistance indexes[fasting insulin(FINS)and insulin resistance(HOMA-IR)]were detected and compared between GDM group and control group.According to the weight gain of GDM gravidas in the mid and late trimester of pregnancy,they were divided into 3 subgroups:insufficient weight gain group(n=30),suitable weight gain group(n=54)and excessive weight gain group(n=28),and the levels of glycolipid metabolism and insulin resistance indexes in each group were compared.Result:HDL-C level in GDM group was lower than that in control group,FPG,HbA1c,TC,TG,LDL-C,FINS and HOMA-IR were higher than those in control group(P<0.05).HDL-C level of excessive weight gain group was lower than that of suitable weight gain group and insufficient weight gain group,FPG,HbA1c,TC,TG,LDL-C,FINS and HOMA-IR were higher than those of suitable weight gain group and insufficient weight gain group(P<0.05).Conclusion:Excessive weight gain in the mid and late trimester of pregnancy can aggravate the disorder of glycolipid metabolism and insulin resistance in gravidas with GDM,so clinical attention should be paid to the weight management of gravidas with GDM in the mid and late trimester of pregnancy.关键词
