基于PET/CT不同勾画方法测量非小细胞肺癌原发灶代谢体积的差异性研究
Study of different delineating methods based on PET/CT for measurement of metabolic parameters in primary tumor of non-small cell lung cancer

Abstract
Objective:To evaluate the impact and repeatability of different PET/CT-based delineating methods for the measurement of metabolic parameters in primary tumor of non-small cell lung cancer(NSCLC).Methods:The data of 79 NSCLC patients who underwent 18F-FDG PET/CT examination were retrospectively analyzed.Maximum standardized uptake value(SUVmax)was measured.The metabolic tumor volume(MTV)and total lesion glycolysis(TLG)were calculated by three methods,the adaptive interative algorithm(AIA)method,the absolute threshold method(2.5,3,4),and the relative threshold method(40%,50%).The differences of metabolic parameters measured by different delineating methods were compared.The interobserver consistency on metabolic parameters were analyzed.Results:MTV was statistically different of different delineating methods(χ2=14.759,P=0.011),while TLG was not(χ2=3.013,P=0.698).For the absolute threshold method and the relative threshold method,the higher the threshold value,the smaller the values of MTV and TLG,and MTV and TLG measured by the relative threshold method were smaller.The repeatability was high for SUVmax,MTV and TLG using three different methods,and the interobserver consistency was high(all ICC>0.99).The relative threshold method(40%)had the highest percentage of without manual adjustment of 18.32%and the highest accuracy of 91.14%,with a significant difference(χ2=11.748,P=0.038).Conclusions:MTV and TLG have the high repeatability from different delineating methods,while MTV are different.The relative threshold method(40%)has the highest percentage of without manual adjustment and the highest accuracy,and is recommended for clinical use.关键词
