Value of diffusion-weighted imaging in clinical evaluation of brain gliomas

目的 分析磁共振扩散加权成像(DWI)在脑胶质瘤临床评估中的价值.方法 选取2020-01-2023-08接受检查的85例脑胶质瘤患者为研究对象,所有患者均接受病理诊断证实,均采用GE 1.5T磁共振成像系统进行检查,对患者进行随访,统计患者复发情况,测定患者肿瘤坏死囊变区、肿瘤周边区、肿瘤实质部分及对侧正常脑白质的表观弥散系数(ADC)值及相对ADC(rADC)值,并进行比较.结果 患者对侧正常脑白质ADC值为(8.32±0.63)×10-4 mm2/s,显著低于肿瘤坏死囊变区、肿瘤周边区及肿瘤实质部分[(25.21±4.27)×10-4 mm2/s、(17.03±3.15)×10-4 mm2/s、(14.33±3.01)×10-4 mm2/s](P<0.05);低级脑胶质瘤的ADC值、rADC值分别为(17.04±2.19)×10-4 mm2/s、1.98±0.22,均显著低于高级脑胶质瘤[(12.89±2.55)×10-4 mm2/s、1.57±0.25](P<0.05);复发患者ADC值为(13.69±1.90)×10-4 mm2/s,显著低于无复发患者的(18.17±2.63)×10-4 mm2/s(P<0.05);ADC值对脑胶质瘤诊断的AUC值、敏感度、特异度分别为0.967、91.76%、100.00%;ADC值对脑胶质瘤不同分级诊断的AUC值、敏感度、特异度分别为0.858、89.19%、64.58%;ADC值对脑胶质瘤预后预测的AUC值、敏感度、特异度分别为0.880、85.29%、82.35%.结论 DWI对脑胶质瘤诊断效果良好,可通过ADC值判断脑胶质瘤分级,预测患者预后.

Objective To analyze the value of diffusion-weighted imaging(DWI)in clinical evaluation of brain gliomas.Methods A total of 85 patients with brain gliomas who received examinations January 2020 to August 2023 were selected as the study subjects.All patients were confirmed by postoperative pathology,and examined by GE 1.5T magnetic resonance imaging system.Follow-up was conducted and recurrence status was analyzed.The apparent diffusion coefficient(ADC)values and relative ADC(rADC)values of tumor necrotic and cystic area,tumor peripheral area,tumor parenchyma and contralateral normal white matter were compared.Results The ADC value of contralateral normal white matter((8.32±0.63)×10-4 mm2/s)was significantly lower than those of tumor necrotic and cystic area,tumor peripheral area and tumor parenchyma((25.21±4.27)×10-4 mm2/s,(17.03±3.15)×10-4 mm2/s and(14.33±3.01)×10-4 mm2/s,respectively,P<0.05).The ADC values and rADC values of low-grade gliomas((17.04±2.19)×10-4 mm2/s,1.98±0.22,respectively)were significantly lower than those of high-grade gliomas((12.89±2.55)×10-4 mm2/s,1.57±0.25,respectively,P<0.05).The ADC value of patients with recurrence((13.69±1.90)×10-4 mm2/s)was significantly lower than that of patients without((18.17±2.63)×10-4 mm2/s,P<0.05).The AUC value,sensitivity and specificity of ADC value for diagnosing brain gliomas were 0.967%,91.76%and 100.00%,respectively.The AUC value,sensitivity and specificity of ADC value for diagnosis of different grades of brain gliomas were 0.858,89.19%and 64.58%,respectively.The AUC value,sensitivity and specificity of ADC value for predicting the prognosis of brain gliomas were 0.880,85.29%and 82.35%,respectively.Conclusion DWI is helpful for diagnosis of brain gliomas.In addition,the ADC value can be used to determine the grade of brain gliomas and predict the prognosis.
