Abstract
Objective:To analyze the effect of percutaneous nephrolithotomy(PCNL)on stone clearance,surgery-related indices,and complications in patients with complex renal stones.Method:80 patients with complex kidney stones admitted to the First People's Hospital of Xianning City from January to December 2023 were selected as the study subjects,and were divided into the experimental group and the control group according to the method of numerical randomization grouping,with 40 cases in each group.Patients in the control group were treated with holmium laser lithotripsy under flexible ureteroscopy,and patients in the test group were treated with PCNL.The surgery-related indexes[lithotripsy time,decreased value of hemoglobin(HB)level,intraoperative bleeding,hospitalization time],complication(hematuria,urinary fistula,fever,urinary tract infection,perirenal injury)incidence,phase Ⅰ stone removal rate,phase Ⅱ surgery rate,as well as improvement of inflammation level and renal function level,were compared between the two groups of patients.Result:The difference in lithotripsy time between the two groups was not statistically significant(P>0.05).The hospitalization time of patients in the test group was shorter than that of the control group,and the difference was statistically significant(P<0.05).The value of decrease in Hb level and intraoperative bleeding of patients in the test group was lower than that of the control group,and the difference was statistically significant(P<0.05).After surgery,all inflammatory indicators and renal function levels of patients in the test group were lower than those of the control group,and the difference was statistically significant(P<0.05).The complication rates of postoperative hematuria,urinary fistula,fever,urinary tract infection and perinephric injury of patients in the test group were lower than those of the control group,and the difference was statistically significant(P<0.05).The rate of one-stage stone removal in the test group was higher than that in the control group,and the rate of second-stage surgery was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion:PCNL can effectively improve the stone removal rate of patients with complex renal stones,which is conducive to reducing the amount of intraoperative bleeding and the incidence of complications,shortening the hospitalization time of patients,and promoting the improvement of patients'inflammation level and renal function level.关键词
