目的 分析慢性炎性脱髓鞘性多发性神经病(CIDP)的磁共振神经成像特征及诊断效率,为疾病诊治提供依据.方法 选取2022-06-2023-09郑州大学第二附属医院收治的72例CIDP患者为观察组,同时选取50例健康体检者为对照组.所有受试者均采用3.0T MRI进行磁共振神经成像(MRN),分析MRN诊断CIDP的效果(敏感性、特异性、阳性预测值、阴性预测值,准确率、误诊率、漏诊率),分析MRN的成像特征及诊断价值.结果 MRN诊断CIDP的敏感性81.94%(59/72),特异性82.00%(41/50),阳性预测值86.76%(59/68),阴性预测值82.00%(41/50),准确率81.97%(100/122),误诊率18.00%(9/50),漏诊率18.06%(13/72).CIDP患者的MRN特征主要表现为神经根增粗,信号增强及强化("袖套征").43例患者臂丛神经(BP)和51例患者腰骶丛神经(LSP)出现增粗肥大,观察组患者BP及LSP神经根直径明显大于对照组(P<0.05),观察组患者马尾神经信号强度高于对照组(P<0.05).结论 MRN主要用于评估CIDP患者周围神经的形态、信号、强化等成像特征,具有准确率高、敏感性强的特点,一定程度上能够反映患者病情,可作为疾病诊断与治疗的辅助检查,具有一定的临床应用价值.

Objective To analyze the MRI features of chronic inflammatory demyelinating polyneuropathy(CIDP)and the diagnostic efficiency,and provide evidence for the diagnosis and treatment of related diseases.Methods Seventy-two patients with CIDP admitted to the Second Affiliated Hospital of Zhengzhou University from June 2022 to September 2023 were selected as the observation group,and 50 healthy people were selected as the control group.All subjects underwent magnetic resonance neuroimaging(MRN)with 3.0T MRI scanner to analyze the effect of MRN in diagnosis of CIDP(sensitivity,specificity,positive predictive value,negative predictive value,accuracy and misdiagnosis rate),and the brachial plexus(BP)and lumbosacral plexus(LSP)neurography of patients were compared,and the imaging characteristics and diagnostic value of MRN were analyzed.Results The sensitivity of MRN in diagnosing CIDP was 81.94%(59/72),the specificity was 82.00%(41/50),the positive predictive value was 86.76%(59/68),the negative predictive value was 82.00%(41/50),the accuracy rate was 81.97%(100/122),and the missed diagnosis rate was 18.06%(13/72).The MRN features of CIDP patients were mainly enhancement of postganglionic signal,enlargement of nerve root,enhancement of preganglionic nerve and postganglionic nerve in intervertebral foramen segment,and"sleeve sign".The brachial plexus of 43 patients(61.3%)and the LS plexus of 51 patients had plexus hypertrophy,including homogeneous hypertrophy and multifocal fusiform hypertrophy.The contrast enhancement of cauda equina in the observation group was significantly stronger than that in the control group(P<0.05).The LS nerve root of brachial artery in the observation group was significantly larger than that in the control group(P<0.05).Conclusion MRN can be used to evaluate the distribution and characteristics of peripheral nerves in patients with CIDP,which can reflect the patient's condition with high accuracy,and can assist in the diagnosis of disease and guide the treatment of patients.It has certain clinical application value.
