Abstract
Objective To analyze the risk factors for poor prognosis in stroke patients with acute anterior circulation stenosis.Methods Stroke patients with acute anterior circulation large vessel stenosis stroke admitted to Anqing 116th Hospital between January 2021 and September 2023 were retrospectively selected.Twenty-three patients with poor prognosis were included in the poor prognosis group after 3 months of follow-up evaluation after treatment,and 46 patients with good prognosis were matched into the good group using propensity matching method.The clinical data of two groups were compared and analyzed,and a risk model was constructed.Results Baseline NIHSS score≥13.705 points,infarct core volume≥52.720 mL,and admission systolic blood pressure≥160.060 mmHg were risk factors for poor prognosis in ALVSS patients.RLMC score≤9.429 is a protective factor for poor prognosis,and has moderate evaluation value for poor prognosis in ALVSS patients(0.7<AUC≤0.9,P<0.05).The risk model constructed by combining risk factors and protective factors has a high predictive value for adverse prognosis in ALVSS patients(AUC>0.9,P<0.05).Conclusion High baseline NIHSS,larger infarct core volume,higher admission systolic blood pressure,and lower rLMC score can indicate poor prognosis in stroke patients with acute anterior circulation macrovascular stenosis.Targeted interventions can be given to improve patient prognosis.关键词
