不同频次氧驱雾化吸入对喉切除病人术后早期气道湿化效果的影响OA
Effect of oxygen aerosol inhalation with different frequencies in early airway humidification after laryngectomy
目的:探讨喉切除病人术后早期最佳的气道湿化频次.方法:选取2020年2月—2022年2月山东省某三级甲等医院耳鼻喉科108例行喉切除术并气管切开的喉癌病人作为研究对象,采用随机数字表法分为3组,每组36例,分别采取每小时1次(A组)、每隔2 h 1次(B组)、每隔3 h 1次(C组)3种频次氧驱雾化吸入进行气道湿化.比较3组病人的气道湿化效果、气道并发症、日吸痰次数及舒适度.结果:喉切除术后前4 d B组的湿化满意度高于A、C两组(P<0.05),术后5~7 d 3组湿化满意度差异无统计学意义(P>0.05);B组刺激性咳嗽的发生率低于A、C两组(P<0.05),其他气道并发症发生率3组比较差异无统计学意义(P>0.05);A组日吸痰次数多于B、C两组(P<0.05);B组的舒适度评分明显高于A、C两组(P<0.05).结论:每隔2 h 1次氧驱雾化吸入能有效提高喉切除病人术后早期气道湿化满意度,降低刺激性咳嗽的发生率,提高病人舒适度,是喉切除术后较为理想的雾化频次.
Objective:To investigate the optimal frequency of airway humidification in early postoperative laryngectomy patients.Methods:A total of 108 laryngeal cancer patients who underwent laryngectomy and tracheotomy in the Otolaryngology Department of a Grade Ⅲ hospital in Shandong province from February 2020 to February 2022 were selected as the study objects,and were divided into 3 groups by random number table method,with 36 cases in each group.Group A received hourly airway humidification,Group B received airway humidification every two hours,and Group C received airway humidification every three hours.The study aimed to compare the effect of airway humidification on airway complications,daily sputum production,and patient comfort among the three groups.Results:The humidification satisfaction of group B was higher than that of group A and Group C at the first 4 days after laryngectomy(P<0.05),and there was no statistically significant difference in humidification satisfaction between groups 5 to 7 days after laryngectomy(P>0.05).The incidence of irritating cough in group B was lower than that in groups A and C(P<0.05),and there was no significant difference in the incidence of other airway complications among the three groups(P>0.05).The number of daily sputum aspiration in group A was higher than that in groups B and C(P<0.05).The comfort score of group B was significantly higher than that of groups A and C(P<0.05).Conclusions:Oxygen spray inhalation every 2 h can effectively improve the satisfaction of early airway humidification,reduce the incidence of irritating cough and improve the comfort of patients,and is an ideal frequency of atomization after laryngectomy.
