目的 观察凯格尔运动辅助经皮低频电刺激对缩短保留神经的宫颈癌根治术后留置导尿管时间的影响.方法 选择2022年5月-2023年3月于复旦大学附属肿瘤医院收治中老年行保留神经的宫颈癌根治术患者42例,采用随机数字表法分为试验组和对照组,每组21例.对照组采用常规导尿管护理联合凯格尔运动干预,试验组采用在与对照组相同干预的基础上加用经皮低频电刺激干预,2组均干预1周.观察并比较2组干预前后残余尿量、导尿管拔除后重新插入率、术后导尿管留置天数、初次排尿时膀胱容量(FD)、膀胱顺应性(BC)、最大膀胱测压容量(MCC).结果 干预后,试验组残余尿量低于对照组[(35.11±6.06)mL vs(46.27±8.14)mL,P<0.05];试验组拔管后导尿管重插率显著低于对照组(19.05%vs 52.38%,P<0.05);试验组术后导尿管留置时间少于对照组[(6.09±1.37)dvs(11.35±2.85)d,P<0.05);试验组BC、MCC值低于对照组,而FD值高于对照组(P<0.05).结论 凯格尔运动辅助经皮低频电刺激可改善保留神经的宫颈癌根治术后患者的排尿功能,减少患者残余尿量及导尿管留置时间,降低拔管后重插率,具有一定的临床应用价值.

Objective To observe the effects of Kegel exercise-assisted percutaneous low-frequency electrical stimula-tion on shortening the indwelling catheterization time after cervical cancer radical surgery with nerve preservation.Methods A total of 42 middle-aged and elderly patients who underwent nerve preservation radical surgery for cervical cancer were selected in Cancer Hospital Affiliated to Fudan University from May 2022 to March 2023.They were randomly divided into experimen-tal group and control group by digital table method,with 21 cases in each group.The control group received conventional cath-eterization nursing combined with Kegel exercise intervention.The experimental group received percutaneous low-frequency electrical stimulation intervention in addition to the same intervention as the control group.Both groups were intervened for one week.The residual urine volume,re-insertion rate after catheter extubation,postoperative catheter retention days,bladder vol-ume at first urination(FD),bladder compliance(BC),and maximal bladder pressure capacity(MCC)were observed and compared between two groups before and after intervention.Results After intervention,the residual urine volume in the ex-perimental group was lower than that in the control group[(35.11±6.06)mL vs(46.27±8.14)mL,P<0.05];the rate of catheter re-insertion after extubation in the experimental group was significantly lower than that in the control group(19.05%vs 52.38%,P<0.05);the postoperative catheter retention time in the experimental group was shorter than that in the control group[(6.09±1.37)d vs(11.35±2.85)d,P<0.05);the BC and MCC values in the experimental group were lower than those in the control group,while the FD value in the experimental group was higher than that in the control group(P<0.05).Conclusion Kegel exercise-assisted percutaneous low-frequency electrical stimulation can improve the urinary function of pa-tients undergoing nerve preservation radical surgery for cervical cancer,reduce residual urine volume and catheter retention time,and lower the rate of re-insertion after extubation.It has certain clinical application value.
