目的 检测血浆长链非编码RNA SOX2-OT在老年慢性阻塞性肺疾病急性加重(AECOPD)相关性肺动脉高压(PH)患者血浆中的表达水平,探讨其潜在的临床应用价值.方法 前瞻性队列研究,纳入120例确诊为AECOPD的老年患者,根据肺动脉收缩压(PASP)分为2组:A组(AECOPD组,60例)和B组(AECOPD相关PH组,60例).在入院时及入院后10 d,评估并比较2组血浆长链非编码RNA SOX2-OT水平、肺动脉收缩压(PASP)、炎症标志物血浆IL-6和IL-8水平、心脏生物标志物血浆NT-ProBNP水平、动脉血气指标值、肺功能指标值、6分钟步行距离测试结果.采用描述性统计分析2组的基线特征,使用t检验或非参数检验比较2组间的差异.并进行相关性分析以评估SOX2-OT与其他指标的关系.结果 在入院时及入院后10 d,2组PASP、血浆长链非编码RNA SOX2-OT水平、血浆IL-6水平、血浆IL-8水平、血浆NT-ProBNP水平、动脉血气指标(pH值、PaO2、PaCO2)值、肺功能指标(FEV1%、DLCO)值、6分钟步行距离差异均有统计学意义(均P<0.05).对B组患者从入院到入院后10 d的指标变化进行比较,发现PASP值、血浆SOX2-OT水平、炎症标志物水平、心脏生物标志物水平、动脉血气指标值、肺功能指标值、6分钟步行距离均显示出时间效应(均P<0.05).在A组和B组中,血浆长链非编码RNA SOX2-OT水平与PASP值、血浆IL-6水平、血浆IL-8水平、血浆NT-ProBNP水平、PaCO2值显示出显著的正相关性(均P<0.05),而与pH值、PaO2值、FEV1%值、DLCO值、6分钟步行距离呈现出显著的负相关性(均P<0.05).结论 合并PH的AECOPD老年患者血浆长链非编码RNA SOX2-OT水平明显高于不合并PH的AECOPD老年患者;AECOPD相关PH患者经过控制感染、纠正呼吸衰竭等治疗后,随着肺动脉压力下降,血浆长链非编码RNA SOX2-OT水平明显下降.

Objective To detect the expression level of plasma long non-coding RNA SOX2-OT in elderly patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)-related pulmonary hypertension(PH),and explore its potential clinical application value.Methods A prospective cohort study included 120 elderly patients diagnosed with AECO-PD.According to their pulmonary artery systolic pressure(PASP),they were divided into two groups:group A(AECOPD group,60 cases)and group B(AECOPD+PH group,60 cases).At admission and 10 days after admission,the levels of plas-ma long non-coding RNA SOX2-OT,inflammation markers(plasma IL-6 and IL-8)and cardiac biomarker(plasma NT-ProB-NP),as well as PASP,arterial blood gas indexes,lung function indexes,and 6-minute walking distance test results were eval-uated and compared between the two groups.Descriptive statistics were used to analyze the baseline characteristics of the two groups,and t-test or nonparametric test was used to compare the differences between the two groups.Correlation analysis was performed to evaluate the relationship between SOX2-OT and other indicators.Results At admission and 10 days after admis-sion,there were statistically significant differences in the levels of plasma long non-coding RNA SOX2-OT,IL-6,IL-8 and plasma NT-ProBNP,and PASP,arterial blood gas indicators(pH,PaO2,PaCO2)values,lung function indicators(FEV1%,DLCO)values,and 6-minute walking distance between the two groups(all P<0.05).The index changes of the group B from admission to 10 days after admission were compared,and it was found that PASP value,plasma SOX2-OT level,inflammation marker level,cardiac biomarker level,arterial blood gas index value,lung function index value and 6-minute walking distance all showed time effects(all P<0.05).In group A and group B,the level of plasma long non-coding RNA SOX2-OT was sig-nificantly positively correlated with PASP value,plasma IL-6 level,plasma IL-8 level,plasma NT-ProBNP level and PaCO2 value(all P<0.05),and negatively correlated with pH value,PaO2 value,FEV1%value,DLCO value and 6-minute walking distance(all P<0.05).Conclusion The level of plasma long non-coding RNA SOX2-OT in AECOPD patients with AECO-PD-related PH is significantly higher than that in AECOPD patients without AECOPD-related PH.After treatment such as infec-tion control and correction of respiratory failure,the level of plasma long non-coding RNA SOX2-OT decreased significantly in patients with AECOPD-related PH as pulmonary artery pressure decreased.
