Abstract
Objective:To investigate the effect of direct anterior approach(DAA)total hip replacement(THA)on elderly patients with intertrochanteric femoral fractures(IFF).Methods:Sixty-four elderly IFF patients admitted to Rizhao Central Hospital from March 2022 to August 2023 were selected as the study objects,and were divided into a control group and a study group according to random number table method,with 32 cases in each group.The control group was treated with traditional posterolateral approach THA,and the study group was treated with DAA-THA.Perioperative indexes,gait indexes,hip function,bone metabolism indexes and complications of the two groups were analyzed.Results:The operation time of the study group was longer than that of the control group,the incision length and hospitalization time were shorter,and the intraoperative blood loss was less than that of the control group,the differences were statistically significant(P<0.05).Six months after surgery,the step frequency of the sthdy group was higher than that of the control group,the step speed was faster than that of the control group,the step length was longer than that of the control group,the scores of Harris Hip Function Scale and osteocalcin were higher than those of the control group,and the tartrate-resistant acid phosphatase-5b and C-terminal peptide of type Ⅰ collagen were lower than those of the control group,with statistical significances(P<0.05).The incidence of complication in the study group was 6.25%,which was lower than 25.00%in the control group,the difference was statistically significant(P<0.05).Conclusion:DAA-THA can improve gait,bone metabolism and hip function and reduce complications in elderly patients with IFF.关键词
