Effects of Question Prompt List in Cancer Patients:A Meta-Analysis
目的:系统评价基于问题提示列表(Question Prompt List,QPL)的沟通干预方法在癌症患者中应用的可行性和效果.方法:计算机检索中国知网、万方、维普数据库、the Cochrane Library、PubMed、Embase及Web of Science数据库中QPL应用于癌症患者的文献,检索时限为1994年1月5日至2023年7月1日.使用Review Manager 5.4软件进行统计分析.结果:最终纳入14篇文献,共2 531例研究对象.Meta分析结果显示,QPL可帮助患者提问(SMD=0.29,95%CI:0.14~0.43,Z=3.87,P<0.001),且不延长咨询时间(SMD=0.02,95%CI:-0.11~0.14,Z=0.26,P=0.800),也不会增加患者的咨询后焦虑(SMD=0.04,95%CI:-0.08~0.17,Z=0.68,P=0.500),并可提高患者在沟通中的自我效能(SMD=0.63,95%CI:0.40~0.86,Z=5.37,P<0.001),患者对咨询的有用性评价较高(SMD=0.30,95%CI:0.03~0.57,Z=2.17,P=0.030).结论:在与癌症患者的沟通过程中,基于QPL的沟通干预方法是可行、有效的.
Objective:To systematically evaluate the feasibility and effects of communication intervention methods based on the Question Prompt List(QPL)in cancer patients.Methods:Literature,covering the period from January 5,1994 to July 1,2023,related to the application of QPL in cancer patients was retrieved from Chinese databases(CNKI,Wanfang and VIP),the Cochrane Library,PubMed,Embase and Web of Science.Statistical analysis was performed using Review Manager 5.4.Results:Fourteen studies,comprising a total of 2,531 study subjects,were included.Meta-analysis results revealed that QPL assisted patients in asking questions(SMD=0.29,95%CI:0.14~0.43,Z=3.87,P<0.001),without extending consultation duration(SMD=0.02,95%CI:-0.11~0.14,Z=0.26,P=0.800);that QPL did not increase post-consultation anxiety in patients(SMD=0.04,95%CI:-0.08~0.17,Z=0.68,P=0.500),but enhanced patients'self-efficacy in communication(SMD=0.63,95%CI:0.40~0.86,Z=5.37,P<0.001);and that patients rated the usefulness of consultations high(SMD=0.30,95%CI:0.03~0.57,Z=2.17,P=0.030).Conclu-sion:Communication intervention methods based on QPL are feasible and effective in the communication process with cancer patients.
