摘要
Abstract
Objective:To explore and analyze the effects of direct anterior approach and posterolateral approach on early gait of patients after total hip replacement.Methods:85 patients with femoral neck fracture treated with total hip re-placement in the Department of Orthopedics of Nantong Second People's Hospital from January 2021 to December 2,2023 were selected and divided into control group(n=42)and test group(n=43).The control group took a direct posterolateral approach, and the test group took a direct anterior approach.Surgical ast between the two groups.Re-sults:At 3 months after surgery, the step speed statistics, step frequency statistics, step length statistics and single support percentage statistics were higher than the control group,P<0.05.In the test group, the Harris hip function score,Berg scale score and Tinetti scale score were higher than that of the control group, and the VAS score value was lower than that of the control group,P<0.05;Serum osteocalcin and 25-hydroxyvitamin D were higher than the control group, and the content of procollagen type Ⅰ was lower than the control group,P<0.05.The intraopera-tive blood loss in the test group was less than that of the control group, the incision length measurement was less than that of the control group, and the hospitalization length and postoperative ambulation were shorter than that of the control group,P<0.05.The postoperative complication rate was lower in the test group than in the control group,P<0.05.Conclusion:with the direct posterolateral approach, the direct anterior approach can reduce the surgical trau-ma,improve the gait, hip function and balance function, relieve postoperative pain, reduce postoperative complications, improve bone metabolism and accelerate the recovery of patients.关键词
