闭环式药学监护模式的构建与实践效果评价
Construction and evaluation of the implementation effectiveness of a closed-loop pharmaceutical care model
摘要
Abstract
AIM To establish an integrated,feedback-driven pharmaceutical care model aimed at delivering superior pharmacy services to hospitalized patients,thereby providing a valuable resource for promoting clinical rational drug use and advancing precision medicine practices.METHODS A robust closed-loop pharmaceutical care service system was intended to be fostered,based on the insights gained from experimenting with pharmaceutical care models.This system would meld the clinical pharmacist department-based model with the medical community's home pharmaceutical services.The success of the closed-loop approach would be assessed by 3 key metrics:the length of hospital stay and the duration of intravenous antibiotic use,the frequency of irrational medical orders,as well as the level of patient satisfaction.RESULTS The model encompasses a comprehensive array of services,including pharmaceutical rounds,pharmaceutical care,pharmaceutical consultations,personalized therapeutic drug monitoring,information counseling,medication education,and home-based pharmaceutical services,to create a cohesive closed-loop pharmaceutical care framework.It had benefited in terms of shortening hospital stays,reducing the duration of intravenous antimicrobial use in inpatients,minimizing inappropriate medical orders,and enhancing satisfaction among medical staff and patients.CONCLUSION This model not only enhances the quality of pharmaceutical services but also improves the patient experience,providing reference for colleagues in implementing standardized and normative pharmaceutical care.关键词
