Efficacy of Nanocomposite Fluid Resin on the Repair of Dental Wedge-shaped Defects

目的:探究Z350纳米复合流体树脂修复牙体楔状缺损的效果及对牙髓反应情况的影响.方法:回顾性分析2022年3月-2023年3月安徽省亳州市利辛县中医院口腔科就诊的牙体楔状缺损患者临床资料,将接受Z350纳米复合流体树脂治疗的35例患者(患牙43颗)纳入观察组,将接受树脂改性光固化玻璃离子材料治疗的31例患者(患牙37颗)纳入对照组.比较两组患者治疗后3个月的修复效果和并发症发生情况(牙齿酸痛、牙髓炎、变色情况);比较两组患者治疗前和治疗后3个月的牙周相关生物标志物[碱性磷酸酶(Alkaline phosphatase,ALP)、天冬氨酸转氨酶(Aspartate aminotransferase,AST)、龈沟液(Gingival crevicular fluid,GCF)]、牙髓反应情况(牙髓电活力、牙髓刺激程度)、咀嚼功能(咀嚼效率、咬合力)和牙齿美观度[正畸治疗需要指数的美观量表(Index of orthodontic treatment need-aesthetic component,IOTN-AC)].结果:观察组的修复有效率显著高于对照组(P<0.05),观察组的并发症总发生率显著低于对照组(P<0.05).治疗后3个月,两组的ALP、AST和GCF均较治疗前升高,但观察组低于对照组(均P<0.05).观察组的牙髓电活力、牙髓刺激程度和IOTN-AC均较治疗前显著降低,且观察组低于对照组(均P<0.05).治疗后3个月,观察组的咀嚼效率、咬合力均较治疗前显著提高(P<0.05),但组间差异无统计学意义(P>0.05).结论:Z350纳米复合流体树脂能有效修复牙体楔状缺损,显著改善患者牙周生物标志物、牙髓反应情况、咀嚼功能和牙齿美观度.

Objective To explore the influence of Z350 nanocomposite fluid resin on the repair effect and pulp reaction status in patients with dental wedge-shaped defects.Methods A retrospective analysis was performed on the clinical data of patients with dental wedge-shaped defects in the department of stomatology of Lixin County Hospital of Traditional Chinese Medicine,Bozhou City,Anhui Province from March 2022 to March 2023.35 patients(43 affected teeth)with Z350 nanocomposite fluid resin were included in observation group,and 31 patients(37 affected teeth)with resin-modified light-cured glass ionomer material were enrolled as control group.The repair effect and occurrence of complications(tooth soreness,pulpitis,discoloration)at 3 months after treatment and relevant periodontal biomarkers[(Alkaline phosphatase,ALP),(aspartate aminotransferase,AST),(Gingival crevicular fluid,GCF)],pulp reaction status(pulp electrical activity,pulp stimulation degree),masticatory function(masticatory efficiency,occlusal force)and dental aesthetics[(Index of orthodontic treatment need-aesthetic component,IOTN-AC)]before treatment and at 3 months after treatment were compared between both groups of patients.Results The effective rate of repair in observation group was significantly higher than that in control group(P<0.05),and the total incidence rate of complications was significantly lower compared to control group(P<0.05).At 3 months after treatment,the levels of ALP,AST and GCF in both groups were significantly enhanced than those before treatment,but the observation group had lower levels(all P<0.05).The pulp electrical activity,pulp stimulation degree and IOTN-AC scores were significantly reduced compared with those before treatment,and the above indicators were lower in observation group(all P<0.05).The masticatory efficiency and occlusal force at 3 months after treatment were obviously risen than those before treatment(P<0.05),but there were no statistical difference between the two groups(P>0.05).Conclusion Z350 nanocomposite fluid resin can effectively repair the dental wedge-shaped defects,and significantly improve the relevant periodontal biomarkers,pulp reaction status,masticatory function and dental aesthetics of patients.
