Abstract
The traditional institutional elderly care model that separates medical care and elderly care can-not meet the needs of the elderly population.Therefore,Yinzhou District government actively guides grassroots health institutions to complement and transform with higher-level hospitals,and strongly supports the construction of an elderly care service system that coordinates home,community,and grassroots health institutions,and com-bines medical care and health care.The Community Health Service Center of Shounan Subdistrict of Yinzhou Dis-trict,in conjunction with the Yagor Elderly Park,utilizes digital medical methods to provide"medical-care led"medical and elderly care integrated services for the elderly in its jurisdiction.This paper introduced their practice and summarized their experience in promoting the transformation and development of grassroots health institutions by providing services for the elderly with geriatric and chronic diseases.关键词
