目的 探讨多发伤合并盆骨骨折患者接受疼痛控制联合基于损伤控制理论的策略的临床效果.方法 选择江苏省苏北人民医院于2021年1月至2023年9月收治的86例多发伤合并盆骨骨折的患者为研究对象,2021年1月至2022年4月入院的患者被纳入对照组(43例),2022年5月至2023年9月入院的患者被纳入观察组(43例).比较2组患者的疼痛程度、舒适度、恢复情况以及并发症发生情况.结果 干预7 d后,2组患者的视觉模拟评分法(VAS)评分均较干预前降低,舒适状况量表(GCQ)评分均较干预前升高;与对照组比较,观察组的VAS评分较低.GCQ评分均较高,上述比较的差异均有统计学意义(P均<0.05).术后28 d后,与对照组比较,观察组患者的术后恢复优良率更高,并发症总发生率更低,差异均有统计学意义(P均<0.05).结论 给予多发伤合并盆骨骨折患者疼痛控制与基于损伤控制理论的策略干预,能够更好地缓解患者的疼痛程度,提高其舒适度,且有利于患者的术后恢复,并降低并发症发生率.

Objective To explore the clinical effect of pain control combined with damage control theory based strategies in patients with multiple injuries and pelvic fractures.Methods A total of 86 patients with multiple injuries combined with pelvic fractures admitted to Northern Jiangsu People's Hospital from January 2021 to September 2023 were selected as the study subjects.Patients admitted from January 2021 to April 2022 were included in the control group(43 cases),and patients admitted from May 2022 to September 2023 were included in the observation group(43 cases).The degree of pain,comfort,recovery and incidence of complications were compared between the two groups.Results After 7 days of intervention,the scores of Visual Analogue Scale(VAS)in both groups decreased compared to before the intervention,and the scores of General Comfort Questionnaire(GCQ)in both groups increased compared to before the intervention.Compared with the control group,the observation group had lower VAS scores and higher GCQ scores,and the differences in the above comparisons were statistically significant(all P<0.05).On the 28th day after surgery,compared with the control group,the observation group had a higher rate of excellent postoperative recovery and lower overall incidence of complications,with statistically significant differences(both P<0.05).Conclusion Providing pain control and strategy intervention based on damage control theory to patients with multiple injuries and pelvic fractures can better alleviate their pain level,improve their comfort,facilitate postoperative recovery,and reduce the incidence of complications.
