摘要

Abstract

Objective:The purposes of the present study were:1)to evaluate intra-and inter-operator reliability of stiffness properties measurements in Achilles tendon(AT)by using the shear wave elastography(SWE);2)to examine the effects of ankle joint angles on the stiffness of AT;3)to com-pare the stiffness of AT in different regions.Methods:Twenty healthy young subjects were recruited from May to August 2018.The average age of male was(23.21±3.09)years old,height was(174.71±5.07)cm,weight was(73.07±13.46)kg,BMI was(23.88±3.85)kg/m2.The average age of fe-male was(20.83±2.23)years old,height was(161.83±5.71)cm,weight was(55.00±13.45)kg,BMI was(20.92±4.48)kg/m2.The two examiners measured the stiffness of 0 cm,3 cm and 6 cm above the calcaneal tubercle of healthy young subjects when the ankle joint was in the relaxed position and 90 degree position with SuperSonic Imagine Aixplorer® Ultrasonic system.The intraclass correlation coefficient(ICC),standard error measure-ment(SEM),minimal detectable change(MDC)and 95%confidence interval(95%CI)were calculated.Paired t-test was used to analyze the effect of different angles on Achilles tendon,and the growth rate of Achilles tendon hardness with ankle angle was calculated.Paired t-test was used to ana-lyze the effect of different angles on AT,and the growth rate of AT stiffness with ankle angle was calculated.Results:The inter-operator reliability of SWE in the measurement of AT at 0 cm(relaxed position)was moderate(ICC=0.66).The intra-operator of AT at 0 cm(neutral position)was excel-lent(ICC=0.91),inter-operator of AT at 3 cm(neutral position)was excellent(ICC=0.93),intra-operator and inter-operator of AT at 6 cm(neutral po-sition)was excellent(ICC=0.96,0.98,respectively).The rest reliability of AT were good.In addition,the MDC and SEM of AT were 14.49~60.28 kPa and 11.87~21.75 kPa,respectively.The stiffness of AT in neutral position was significantly higher than that in relaxed position(P<0.001).When the ankle angle was changed from relaxed position to neutral position,the stiffness of AT changed as follows:AT0cm>AT3cm>AT6cm,and the stiff-ness growth rates were 33.06%±16.49%,28.34%±15.31%and 28.21%±14.33%,respectively.Conclusion:SWE has comparatively high reliability in quantifying the stiffness of Achilles tendon whatever AT were relaxed or stretched.The stiffness of AT is related to the position of the ankle joint.SWE can provide the objective data of AT to clinicians for further diagnosis and treatment.
