Analysis of the status and influencing factors of caregivers'participation in hospice doctor-patient shared decision-making for patients with advanced cancer

Abstract
Objective To investigate the current status and influencing factors of primary caregivers'participation in hospice physician-patient shared decision-making in patients with advanced cancer.Methods A convenience sampling method was used to select 420 primary caregivers of advanced cancer patients who were treated at a tertiary-level general hospital in Wuxi City,from June 2023 to December 2023.The survey was conducted using the General Information Questionnaire,Decision Preferences,Decision Preparation,Patient Version of the Shared Decision Making Scale,and the Decision Conflict Scale.Results The total mean score of primary caregiver participation in hospice doctor-patient shared decision-making for patients with advanced cancer 35.05±5.24.The total mean score for decision preparation was 53.48±14.34,and the total mean score for decision conflict was 56.10±15.70.The level of joint decision-making was positively correlated with decision preference and decision readiness(r=0.213,0.123,P＜0.001)and negatively correlated with decision conflict(r=-0.326,P＜0.001);and negatively correlated with decision readiness and decision conflict(r=-0.118,P＜0.001).Multiple linear regression analyses revealed that patients'healthcare payment method,primary caregiver literacy level,marital status,occupation,decision-making preference,decision-making readiness,and decision-making conflict were influencing factors of their participation in hospice doctor-patient shared decision-making.Conclusions The participation of primary caregivers of patients with advanced oncology in hospice doctor-patient shared decision-making is at a moderate level,with inadequate preparation for decision-making and conflicting decisions.Health care professionals should improve primary caregivers'decision-making readiness and reduce decision-making conflicts through various means such as decision support tools based on their decision-making preference styles,to increase their level of participation in hospice doctor-patient shared decision-making,improve the quality of decision making,and promote hospice implementation.关键词
