目的:观察肌肉能量技术联合体外冲击波治疗慢性非特异性下腰痛的疗效.方法:纳入45例慢性非特异性下腰痛患者并随机分为观察组22例和对照组23例,对照组采用药物、推拿等常规治疗,观察组在常规治疗基础上采用肌肉能量技术联合体外冲击波治疗,比较两组治疗3周后和12周随访的疼痛评分(VAS)、表面肌电图均方根值(RMS)以评估患者从坐位到站立位腹横肌、多裂肌的肌肉激活水平,以及Oswestry功能障碍指数(ODI).结果:治疗3周后,对照组VAS、ODI评分显著降低(P<0.05),腹横肌和多裂肌RMS值与治疗前相比差异无统计学意义(P>0.05),观察组各项指标则均有显著改善(P<0.05);12周后随访,观察组各项指标均显著优于对照组(P<0.05).结论:肌肉能量技术联合体外冲击波可以明显缓解疼痛,提高腰腹力量,改善慢性非特异性下腰痛患者的腰部功能,而且疗效较为持久.

Objective:To observe the efficacy of muscle energy technology combined with extracorporeal shock wave in chronic nonspecific low back pain.Methods:Forty-five patients with chronic non-specific lower back pain were enrolled and randomly divided into 22 cases in the observa-tion group and 23 cases in the control group.The control group was treated with conventional treatments included drugs and massage,while the ob-servation group was treated with muscle energy technology combined with extracorporeal shock wave therapy on top of the conventional treatments,and the two groups were compared in terms of the pain scores(VAS),the root-mean-square(RMS)values of surface electromyography at the 3-week and 12-week follow-up visits in order to assess the activation level of the abdominal transversus abdominalis muscle,the multifidus muscle in the pa-tients from the seated position to the standing position,as well as the Oswestry Dysfunction Index(ODI).Results:After 3 weeks of treatment,the VAS and ODI scores of the control group were significantly decreased(P<0.05),and the differences in the RMS values of transversus abdominis muscle and multifidus muscle were not statistically significant compared with the pre-treatment values(P>0.05),while all the indexes of the observa-tion group improved significantly(P<0.05);at the follow-up visit after 12 weeks,the indexes of the observation group were significantly better than those of the control group(P<0.05).Conclusion:Muscle energy technology combined with extracorporeal shock wave can significantly relieve pain,improve lumbar and abdominal strength,and improve lumbar function in patients with chronic nonspecific lower back pain,and the efficacy is more durable.
