股骨颈动力交叉钉系统内固定术与空心加压螺钉内固定术治疗股骨颈骨折患者的效果分析OA
Effect Analysis of Femoral Neck Dynamic Cross-Nail System Internal Fixation and Cannulated Compression Screw Internal Fixation in the Treatment of Patients with Femoral Neck Fracture
目的:探讨股骨颈动力交叉钉系统(FNS)内固定术与空心加压螺钉(CCS)内固定术治疗股骨颈骨折患者的临床效果.方法:选取 2020年 12月—2023年 1月盐城市大丰人民医院收治的 44例股骨颈骨折患者为研究对象,按照随机数字表法将患者分为对照组与研究组,各22例.对照组采用CCS内固定术,研究组采用FNS内固定术.比较两组的围手术期指标、并发症发生情况、治疗结局情况、生活独立能力、髋关节功能及日常生活活动能力.结果:研究组手术时间、住院时间、术后完全负重时间均短于对照组,术中透视次数、术中出血量均少于对照组,差异均有统计学意义(P<0.05).研究组并发症发生率为 9.09%,低于对照组的 36.36%,差异有统计学意义(P<0.05).研究组骨折愈合时间、股骨颈缩短长度均短于对照组,颈干角变化小于对照组,差异均有统计学意义(P<0.05).研究组大腿外侧激惹发生率为 4.55%,低于对照组的36.36%,差异有统计学意义(P<0.05).术后3、6个月,研究组功能独立性评定量表、Harris髋关节功能评分量表及日常生活活动能力量表评分均高于对照组,差异均有统计学意义(P<0.05).结论:FNS与CCS内固定术在股骨颈骨折患者的治疗中均具有较好治疗效果,但FNS内固定术围手术期指标均优于CCS内固定术,且术后并发症发生率低,患者出院后随访结局理想,有利于改善患者预后.
Objective:To investigate the clinical effect of femoral neck dynamic cross-nail system(FNC)internal fixation and cannulated compression screw(CCS)internal fixation on patients with femoral neck fracture.Methods:Forty-four patients with femoral neck fracture admitted to Yancheng Dafeng People's Hospital from December 2020 to January 2023 were selected as the study objects.According to random number table method,the patients were divided into a control group and a study group,with 22 cases in each group.CCS internal fixation was used in the control group,FNC internal fixation was used in the study group.Perioperative period indexes,occurrence of complications,treatment outcomes,independence of life,hip function and activities of daily living were compared between the two groups.Results:The operation time,hospital stay and total weight bearing time of the study group were shorter than those of the control group,and the intraoperative fluoroscopy times and intraoperative blood loss were less than those of the control group,with statistical significances(P<0.05).The incidence of complications in the study group was 9.09%,which was lower than 36.36%in the control group,the difference was statistically significant(P<0.05).The fracture healing time and shortening length of femoral neck in the study group were shorter than those in the control group,and the changes of cervical trunk angle were smaller than those in the control group,with statistical significances(P<0.05).The incidence of lateral thigh irritation in the study group was 4.55%,which was lower than 36.36%in the control group,the difference was statistically significant(P<0.05).At 3 and 6 months after operation,the scores of Functional Independence Measure Scale,Harris Hip Function Scale and Activities of Daily Living Scale in the study group were higher than those in the control group,with statistical significances(P<0.05).Conclusion:Both FNC and CCS have good therapeutic effects in the treatment of patients with femoral neck fracture,but FNC internal fixation has better perioperative period index therapeutic effect than CCS internal fixation,and the incidence of postoperative complications is low,and the follow-up outcome of patients after discharge is ideal,which is conducive to improving the prognosis of patients.
