局部晚期鼻咽癌患者诱导化疗联合同期放化疗治疗前后外周血免疫相关指标的变化和意义
Changes in and Significance of Peripheral Blood Immunity-Related Inde-xes before and after Induction Chemotherapy Combined with Concurrent Chemoradiotherapy in Patients with Locally Advanced Nasopharyngeal Carcinoma

Abstract
Objective:To explore and analyze the changes in and significance of peripheral blood immunity-related inde-xes before and after induction chemotherapy combined with concurrent chemoradiotherapy in patients with locally advanced nasopharyngeal carcinoma.Methods:We retrospectively analyzed the peripheral blood immunity-related indexes of 48 pa-tients with locally advanced nasopharyngeal carcinoma admitted to our hospital from December 2019 to July 2021.PD-1,CT-LA-4,Treg,CD4+/CD8+,CD3+,CD4+,CD16+,plasmacytoid dendritic cells(PDC)and myeloid dendritic cells(MDC)in the peripheral blood were detected before treatment,after induction chemotherapy,and after concurrent chemora-diotherapy.The changes in the percentages of the above indicators before treatment,after induction chemotherapy and after chemoradiotherapy were compared.The treatment regimen of the included patients was induction chemotherapy combined with concurrent chemoradiotherapy.Results:CTLA-4 and Treg after induction chemotherapy were significantly lower than those before treatment(P<0.05);and there was a downward trend in CD 16+and PDC after induction chemotherapy(P>0.05);CD3+and CD4+after induction chemotherapy were significantly higher than those before treatment(P<0.05);and there was an upward trend in PD-1,CD4+/CD8+and MDC after induction chemotherapy(P>0.05).Treg,CD4+/CD8+,CD3+,CD4+and CD16+after chemoradiotherapy were significantly lower than those before treatment(P<0.05);there were a downward trend in CTLA-4 and an upward trend in MDC after chemoradiotherapy(P>0.05);and PD-1 and PDC significantly increased after chemoradiotherapy(P<0.05).After induction chemotherapy combined with concurrent chemo-radiotherapy,the expression of PDC in the CR group was significantly higher than that in the PR group(P<0.05);the ex-pressions of CD3+,CD4+,CD 16+and CD4+/CD8+in the CR group were slightly lower than those in the PR group(P>0.05);the expressions of PD-1,Treg,CTLA-4 and MDC in the CR group were slightly higher than those in the PR group(P>0.05).Conclusion:Induction chemotherapy combined with concurrent chemoradiotherapy regulates immune-related in-dexes such as PD-1,CTLA-4,Treg and PDC in peripheral blood of locally advanced nasopharyngeal carcinoma patients.The anti-tumor mechanism of chemoradiotherapy is not only related to the direct killing of tumor cells by chemoradiotherapy,but also related to the regulation of PD-1,CTLA-4,Treg,PDC and other indicators in the tumor microenvironment.Changes in the above indicators in peripheral blood may predict the treatment effect,and determine whether to carry out immunotherapy after treatment.关键词
