目的 观察基于静息态功能磁共振成像检查的首发未治疗青少年女性抑郁症患者大脑功能活动变化,并分析其与认知功能的关系.方法 纳入首发未治疗的青少年女性抑郁症患者30例(观察组)及健康对照24例(对照组).两组均进行静息态功能磁共振检查,获取平均低频振幅(mALFF)值并比较;然后取两组mALFF值有差异的脑区中与认知功能关系密切的扣带回和额中回作为感兴趣区域,采用基于感兴趣区域的功能连接分析方法观察两组之间的大脑功能活动,并获取功能连接值.两组均进行重复性成套神经心理状态评定量表(RBANS)调查,评估认知功能.采用Pearson相关分析两组mALFF值差异性脑区mALFF、FC值与认知功能的相关性.结果 与对照组相比,观察组右侧额中回、右侧内侧额上回、左侧扣带回、右侧角回的mALFF降低(P均<0.05).基于扣带回作为感兴趣区域做全脑的功能连接分析结果:与对照组相比,观察组的右侧海马旁回和右侧眶部额下回的功能连接值增高(P<0.05).基于额中回作为感兴趣区域做全脑功能连接分析结果:与对照组相比,观察组双侧小脑8区和左侧补充运动区的功能连接值增高(P均<0.05).观察组认知功能与差异性脑区的mALFF和功能连接值无相关性(P均>0.05).两组重复性成套神经心理状态评定量表(RBANS)总分、即刻记忆、视觉广度、言语功能、延时记忆评分比较,P均<0.05.结论 首发未治疗青少年女性抑郁症患者扣带回、额中回等关键脑区mALFF、功能连接存在异常,但并未发现上述脑区功能活动与认知功能存在相关性.

Objective To observe brain functional activities in first-episode,untreated adolescent female patients with depression based on the resting-state functional magnetic resonance imaging(rs-fMRI)and to analyze its relationship with cognitive function.Methods Thirty first-episode,untreated adolescent female patients with depression(observa-tion group)and 24 healthy controls(control group)were included.Both groups underwent rs-fMRI to obtain the amplitude of low-frequency fluctuation(ALFF)values,which were compared between the two groups.Regions of interest(ROIs)as-sociated with cognitive function,such as the cingulate gyrus and middle frontal gyrus,were selected from the brain regions showing differences in ALFF values between the two groups.Functional connectivity(FC)analysis was performed based on these ROIs to observe brain functional activity between the two groups and to obtain FC values.Both groups were also assessed for cognitive function using the Repeatable Battery for the Assessment of Neuropsychological Status(RBANS).Pearson correlation analysis was used to assess the relationships between the ALFF and FC values of the brain regions show-ing differences in ALFF and cognitive function.Results Compared with the control group,the observation group exhibit-ed decreased ALFF in the right middle frontal gyrus,right medial superior frontal gyrus,left cingulate gyrus,and right an-gular gyrus(all P<0.05).Whole-brain functional connectivity analysis based on the cingulate gyrus as the ROI showed higher FC values in the right parahippocampal gyrus and right orbital inferior frontal gyrus in the observation group in com-parison with those of the control group(all P<0.05).Whole-brain functional connectivity analysis based on the middle frontal gyrus as the ROI revealed increased FC values in the bilateral cerebellum area 8 and left supplementary motor area in the observation group in comparison with those of the control group(both P<0.05).There were no correlations between cognitive function and ALFF or FC values in the brain regions with differences(all P>0.05).There were significant differ-ences in the RBANS total score,immediate memory,visual span,language function,and delayed memory scores between the two groups(all P<0.05).Conclusion First-episode,untreated adolescent female patients with depression exhibited abnormalities in ALFF and functional connectivity in key brain regions such as the cingulate gyrus and middle frontal gy-rus,but no correlation was found between brain functional activity and cognitive function in these regions.
