非透析慢性肾脏病患者睡眠障碍影响因素分析
Analysis of influencing factors of sleep disorders in non-dialysis patients with chronic kidney disease

Abstract
Objective To explore the sleep disorders and its influencing factors in patients with chronic kidney disease(CKD).Methods A total of 406 CKD patients with non-dialysis stages 1-5 were collected,who were hospitalized in the Department of Nephrology at Central Hospital of Dalian University of Technology from June 2022 to December 2023.They were evaluated for sleep disorders according to the Pittsburgh Sleep Quality Assessment Scale,and then they were divided into normal sleep group(233 cases)and sleep disorders group(173 cases).The t test,Mann-Whitney U test,and Chi-square test were used for inter-group comparison,and univariate and multivariate logistic regression models were used to analyze the influencing factors of sleep disorders.Results Among the 406 patients,42.61％of CKD patients suffered from sleep disorders.Univariate analysis showed that urea nitrogen,blood creatinine,estimated glomerular filtration rate(eGFR),blood potassium,anxiety,depression,and disease stage were the influencing factors of sleep disorders in CKD patients.Multivariate logistic regression analysis suggested that blood potassium(OR=0.568,95％CI 0.341-0.948)was an independent protective factor for sleep disorders,while anxiety(OR=1.735,95％CI 1.057-2.846)was an independent risk factor for sleep disorders.Subgroup analysis showed that anxiety(OR=1.841,95％CI 1.175-2.884)and systolic blood pressure(OR=1.034,95％CI 1.005-1.064)were risk factors respectively for sleep disorders in patients with CKD stages 1-3 and CKD stages 4-5.Conclusion Anxiety and higher systolic blood pressure may be the main risk factors for sleep disorders in CKD patients,and maintaining reasonable blood potassium levels may reduce the risk of sleep disorders in the patients.关键词
