基于体素形态学多维度分析系统性红斑狼疮患者脑实质体积变化的研究OA北大核心CSTPCD
Multidimensional voxel-based morphometric analysis of brain parenchymal volume variations in patients with systemic lupus erythematosus
目的 通过基于体素的形态学分析(VBM)方法探究系统性红斑狼疮(SLE)患者脑实质体积的改变.方法 按照性别和年龄1∶1配对的方式纳入2007-2021年昆明医科大学第一附属医院风湿免疫科常规头颅磁共振扫描正常的SLE患者及同期健康对照(HCs)各85例,对其3D-T1脑结构磁共振图像分割并计算全脑总体积、脑区平均体积和体积脑图,从多个维度进行比较.结果 SLE组相较于HCs组,全脑水平下,灰质体积明显减少(P<0.05);脑区水平下,双侧梭状回、脑岛等15个脑区体积明显减小(经FDR校正,q<0.05);团块水平下,灰质区域右直回体积减少(GRF校正,Voxel P<0.001,Cluster P<0.01).结论 SLE患者表现出多个维度的脑体积下降,采用VBM方法可在常规磁共振扫描正常的SLE患者中发现脑实质的萎缩.
Objective To investigate alterations in brain parenchymal volume in patients with systemic lupus erythematosus(SLE)using the voxel-based morphometry(VBM)approach.Methods SLE patients with normal routine brain magnetic resonance imaging(MRI)and healthy controls(HCs)in Department of Rheumatology and Immunology,First Affiliated Hospital of Kunming Medical University from 2007 to 2021 were recruited at a 1∶1 matching ratio based on gender and age.The 3D-T1 brain structural magnetic resonance images were segmented and the total brain volume,average brain region volume and voxel brain map were calculated,which were compared from multiple dimensions.Results At the whole brain level,the gray matter volumes of SLE group were significantly reduced compared with those of the HCs group(P<0.05).At the level of brain regions,the volume of 15 brain regions such as the bilateral fusiform gyrus and insula decreased significantly(after FDR correction,q<0.05).At the cluster level,the volume of the right rectus in the gray matter area decreased(GRF correction,Voxel P<0.001,Cluster P<0.01).Conclusion SLE patients exhibit brain volume reduction at multiple dimensions.VBM methods can detect brain parenchymal atrophy in SLE patients with normal MRI.
