Abstract
Objective:To investigate the perioperative indexes and effects on neck function of endoscopic gasless radical thy-roidectomy for unilateral thyroid cancer in transaxillary approach.Methods:From Jan.2020 to May 2022,120 patients with unilateral thyroid cancer were selected and divided into control group(n=60,traditional open surgery)and study group(n=60,endoscopic gas-less radical thyroidectomy in axillary approach)based on different surgical method.Clinical data of the patients were collected and the operative indicators,inflammatory indicators,neck function,and incidence of postoperative complications were compared between the two groups.Results:The intraoperative blood loss,postoperative drainage volume,postoperative hospital stay,neck pain visual analogue score,neck injury index score,and swallowing dysfunction index score of the study group were less than those of the control group,and the surgical time was longer than that in the control group,with statistically significant difference(P<0.05).At 2 h preoperatively,there was no statistically significant difference in the levels of interleukin-6 and tumor necrosis factor-α between the two groups(P>0.05);at24 h postoperatively,the level of interleukin-6 and tumor necrosis factor-α of the two groups significantly reduced,and the study group was lower than the control group(P<0.05).The total incidence of complications in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).At3 months postoperatively,the total cosmetic satisfaction of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusions:Endoscopic gasless radical thyroidectomy for thyroid cancer in axillary approach could improve operative indicators and neck function in patients,decrease the level of inflammatory factors and the incidence of postoperative complications,and increase cosmetic satisfaction.关键词
