改良股骨近端抗旋髓内钉固定手术切口治疗股骨粗隆间骨折的疗效观察
Therapeutic effect of modified PFNA incision on intertrochanteric fracture of femur

Abstract
Objective To investigate the feasibility,perioperative period and postoperative follow-up of modified proximal femoral nail antirotation(PFNA)incision in the treatment of intertrochanteric fracture of femur.Methods The data of 46 patients undergoing PFNA surgery for intertrochanteric fracture of femur in Maanshan General Hospital of Ranger-Duree Healthcare from May 2021 to May 2022 were retrospectively analyzed,modified PFNA incision and traditional PFNA incision were selected for the treatment of patients with intertrochanteric fracture of femur.Patients were divided into the modified incision group(n=27)and the traditional incision group(n=19)according to different surgical incisions.The operation time,intraoperative blood loss,incision length,intraoperative fluoroscopy times,average length of hospital stay of patients in the two groups,as well as WOMAC,VAS scores and imaging data of patients at 1 day,1 month and 1 year postoperative follow-up were compared.The incidence of complications in the two groups was monitored.Results All 46 patients were followed up for 1 year.The modified incision group had shorter operation time,less intraoperative blood loss,shorter incision length,less intraoperative fluoroscopy times,and shorter mean hospital stay,and the differences were statistically significant(P<0.05).There was no statistical significance in the WOMAC and VAS scores between the two groups at 1 d,1 month and 1 year after surgery(P>0.05).During postoperative follow-up,there was 1 case of delayed fracture union in the modified incision group and 1 case of screw blade denailing in the traditional incision group during X-ray imaging evaluation,with no statistically significant difference(P>0.05).Conclusion The modified PFNA incision for the treatment of intertrochanteric fracture of femur is safe and reliable,with significantly reduced operation time and blood loss,less skin pulling during guide needle insertion,shorter incision length,and less fluoroscopy times during surgery.The modified incision may make the treatment of intertrochanteric fracture of femur with PFNA easier and more effective.关键词
