纤维支气管镜支气管肺泡灌洗术治疗难治性肺炎支原体肺炎的效果OA
Effect of Fiberoptic Bronchoscopy Bronchoalveolar Lavage in the Treatment of Refractory Mycoplasma Pneumoniae Pneumonia
目的:评估纤维支气管镜支气管肺泡灌洗术(BAL)应用在难治性肺炎支原体肺炎(RMPP)患儿中的效果.方法:纳入 2021 年 1 月—2023 年 8 月湛江中心人民医院收治的 60 例RMPP患儿作为研究对象,遵循随机数表法将患儿分为对照组(n=30,给予常规药物治疗)与观察组(n=30,在对照组基础上加用BAL).比较两组临床疗效、炎症因子[白细胞计数(WBC)、降钙素原(PCT)、肿瘤坏死因子-α(TNF-α)]、肺功能[最大呼气中期流速(MMEF)、呼气峰值流量(PEF)、用力肺活量(FVC)]及康复相关时间.结果:较对照组,观察组临床总有效率更高,康复相关时间更短,差异有统计学意义(P<0.05);较对照组,观察组治疗 14 d后WBC、PCT、TNF-α更低,MMEF、PEF、FVC更高,差异有统计学意义(P<0.05).结论:RMPP患儿实施纤维支气管镜BAL能提高整体疗效,调节炎症因子水平,改善肺功能,缩短康复时间.
Objective:To evaluate the effect of fiberoptic bronchoscopy bronchoalveolar lavage(BAL)in children with refractory mycoplasma pneumoniae pneumonia(RMPP).Method:A total of 60 children with RMPP who admitted to Central People's Hospital of Zhanjiang from January 2021 to August 2023 were included in the study,the children were divided into the control group(n=30,routine drug treatment)and the observation group(n=30,BAL on the basis of control group)according to the random number table method.The clinical efficacy,inflammatory factors[white blood cell count(WBC),procalcitonin(PCT),tumor necrosis factor-α(TNF-α)],lung function[maximum mid-expiratory flow rate(MMEF),peak expiratory flow(PEF),forced vital capacity(FVC)]and rehabilitation related time were compared between two groups.Result:Compared with the control group,the clinical effective rate of the observation group was higher and the rehabilitation related times were shorter,the difference was statistically significant(P<0.05).Compared with the control group,the WBC,PCT and TNF-α in the observation group after 14 d of treatment were lower,the MMEF,PEF and FVC were higher,and the difference was statistically significant(P<0.05).Conclusion:The implementation of fiberoptic bronchoscopy BAL in children with RMPP can improve the overall efficacy,regulate the level of inflammatory factors,improve lung function,and shorten the rehabilitation time.
