不同波形参数电针治疗中风后吞咽困难的疗效分析OA
Efficacy Analysis of Electroacupuncture with Different Waveform Parameters in Treating Post-Stroke Dysphagia
目的:探讨不同波形参数的电针治疗中风后吞咽困难的临床疗效差异.方法:选择符合纳入标准的60例中风后吞咽困难患者,依据随机数字表随机分为疏密波组(2/100 Hz)和疏波组(2 Hz),每组各30例.参照中国针灸学会发布的《循证针灸临床实践指南》,选择廉泉、夹廉泉、风池、供血、翳风、列缺、照海、通里和内关穴,分别给予不同波形参数进行电针治疗,每日1次,每周治疗6 d,两组均治疗4周.采用洼田饮水试验及标准吞咽功能评定量表(SSA量表)比较两组患者治疗前、治疗2周及治疗4周后的吞咽功能变化,并比较临床疗效.结果:疏密波组临床总有效率为93.33%(28/30),疏波组临床总有效率为76.67%(23/30),两组比较差异有统计学意义(P<0.05).与治疗前比较,治疗2周和4周后两组患者洼田饮水试验分级评分、SSA评分均降低(P>0.05);与疏波组患者治疗4周后比较,疏密波组洼田饮水试验分级评分、SSA评分降低(P<0.05).结论:2/100 Hz疏密波与2 Hz疏波治疗方案均能改善患者的吞咽功能,但疏密波远期疗效优于疏波.
Objective:To explore the clinical efficacy differences of electroacupuncture with different waveform parameters in treating post-stroke dysphagia.Methods:Sixty patients with post-stroke dysphagia meeting the inclusion criteria were randomly divided into two groups using a random number table:the sparse-dense wave group(2/100 Hz)and the sparse wave group(2 Hz),with 30 patients in each group.Based on the"Evidence-Based Acupuncture Clinical Practice Guidelines"published by the Chinese Acupuncture Society,acupuncture points including Lianquan(LI4),Jialianquan(LI11),Fengchi(GB20),Gongxue(ST36),Yifeng(TE5),Lieque(LU7),Zhaohai(KD6),Tongli(PC8),and Neiguan(PC6)were selected.Different waveform parameters were applied in electroacupuncture treatment,administered once daily,6 days a week,for 4 weeks.The changes in swallowing function were assessed using the Water Swallowing Test and the Standard Swallowing Assessment(SSA)scale before treatment,after 2 weeks,and after 4 weeks.Clinical efficacy was compared between the two groups.Results:The total clinical effective rate was 93.33%(28/30)in the sparse-dense wave group and 76.67%(23/30)in the sparse wave group,with a statistically significant difference(P<0.05).Compared to before treatment,the Water Swallowing Test scores and SSA scores decreased in both groups after 2 and 4 weeks of treatment(P>0.05).The sparse-dense wave group showed a more significant reduction in Water Swallowing Test scores and SSA scores after 4 weeks compared to the sparse wave group(P<0.05).Conclusion:Both the 2/100 Hz sparse-dense wave and 2 Hz sparse wave treatment protocols can improve swallowing function in patients,but the sparse-dense wave shows superior long-term efficacy compared to the sparse wave.
