术中超声在脑胶质瘤切除术中应用价值的Meta分析OACSTPCD
Meta-Analysis about the Value of Intraoperative Ultrasound in the Glioma Resection Surgery
目的:系统评价术中超声(intraoperative ultrasound,IOUS)引导脑胶质瘤切除的全切率、手术时间及预后.方法:在中国知网、万方、维普、中国生物医学文献数据库、PubMed、Web of Science、Embase、The Cochrane Library搜索关于IOUS在脑胶质瘤切除中应用的文献,检索时间为建库至2022年12月.由两名评价者独立对纳入文献进行严格评价和资料提取后,采用Review Manager 5.4软件进行Meta分析.结果:最终纳入16篇文献,共1 573个病例.在全切率、手术时间、1年生存率和2年生存率方面,IOUS组与对照组的差异有统计学意义(RR=1.41,95%CI:1.27~1.56,P<0.001;SMD=-0.56,95%CI:-0.74~-0.38,P<0.001;RR=1.35,95%CI:1.19~1.53,P<0.001;RR=1.63,95%CI:1.34~2.00,P<0.001).结论:IOUS引导脑胶质瘤切除术能提高全切率、缩短手术时间及提高1年生存率和2年生存率.
Objective:To systematically evaluate the total resection rate and operation time of glioma resection surgery guided by intraoperative ultrasound(IOUS),and the prognosis of patients.Methods:The literature about the application of IOUS in the resection of glioma was searched from the databases including CNKI,Wan Fang,VIP,CBM,PubMed,Web of Sci-ence,Embase and the Cochrane Library from the establishment of the database to December 2022.After rigorous evaluation and data extraction by two evaluators independently,meta-analysis was conducted by Review Manager 5.4.Results:Sixteen articles with a total of 1,573 cases were included.In the total resection rate,surgical duration,l-year survival rate,and 2-year survival rate,there were statistically significant differences between the IOUS group and the control group(RR=1.41,95%CI:1.27~1.56,P<0.001;SMD=-0.56,95%CI:-0.74~-0.38,P<0.001;RR=1.35,95%CI:1.19~1.53,P<0.001;RR=1.63,95%CI:1.34~2.00,P<0.001).Conclusion:Glioma resection surgery guided by IOUS can improve the total resection rate,shorten the surgical duration,and improve the 1-and 2-year survival rates of the patients.
