芒硝外敷在复杂胫骨平台骨折术前肿痛的临床研究
Clinical Study of External Application of Ammonium Nitrate for Preoperative Swelling and Pain in Complex Tibial Plateau Fractures

Abstract
Objective:To evaluate the application effect of external application of ammonium nitrate in preoperative swelling and pain of complex tibial plateau fractures.Methods:A total of 62 patients with complex tibial plateau fractures were randomly divided into a control group and an obser-vation group,with 31 patients in each group.After admission,all patients were instructed to perform isometric contraction of the quadriceps femoris and ankle joint flexion-extension functional exercise.The control group received intravenous infusion of 200 mL mannitol once daily in combination with intravenous infusion of 15 mg sodium aescinate once daily.The observation group received external application of ammonium nitrate around the knee joint in addition to the treatment in the control group,with a duration of 12 hours per day.The two groups were observed for limb swelling score,visual analogue scale(VAS)score for limb pain,inflammatory markers,hemorheological parameters,complications,and safety analysis of ex-ternal application of ammonium nitrate.Results:After treatment,the VAS score,affected limb swelling score,C-reactive protein value,IL-6,blood viscosity high cut value,blood viscosity low cut value and plasma viscosity value of the patients in both groups were lower than those before treat-ment(P<0.05),and the VAS score,affected limb swelling score,C-reactive protein value,IL-6,blood viscosity high cut value,blood viscosity low cut value and plasma viscosity values were lower than those of the control group(P<0.05).In terms of fracture complications,no patients in the two groups had osteofascial compartment syndrome,and there were 3 cases and 8 cases of lower limb venous thrombosis and tension blisters in the con-trol group,and 1 case and 2 cases in the observation group respectively,and the incidence of complications in the observation group was significantly lower than that in the control group(P<0.05).Conclusion:Compared with the intravenous infusion of mannitol and sodium aescinate alone,the com-bined application of external ammonium nitrate is more effective in relieving preoperative swelling and pain in patients with complex tibial plateau fractures,reducing fracture complications,and is safe,indicating its potential for widespread use.关键词
