超重对膝骨性关节炎患者登梯下肢生物力学的影响
The Effect of Overweight on Lower Limb Biomechanics during Stair Climbing in Patients with Knee Osteoarthritis

Abstract
Objective:To observe the difference of spatial-temporal parameters and kinetic parameters between overweight knee osteoarthritis(KOA)patients and normal weight KOA patients by three-dimensional gait analysis,and to analyze the effect of overweight on the lower limb biomechanics in KOA patients.Methods:According to body mass index(BMI),32 cases of KOA patients were divided into normal weight group(18.5≤BMI<24)and overweight group(24≤BMI<28).Each group consisted of 16 cases,and gait analysis was done to assess the temporo-spatial parameters and ki-netic variables during stair climbing.Results:Compared with normal weight group,the stride length,foot angle,maximal ankle plantar flexor mo-ments were greater during ascending stairs(P<0.05);the maximal knee extensor and flexor moments,and the first peak external knee adduction mo-ment(PEKAM1)as well were significantly increased during descending stairs(P<0.05)in overweight group.Conclusion:Obesity is a risk factor of KOA,overweight cases have present increased joint loading in the lower limb than cases with normal weight,mainly in the knee and ankle kinetics,especially during descending stairs,indicating the importance of weight control to reduce joint load and delay the progression of the disease.关键词
