胰岛素溶液间断灌洗联合封闭式负压引流治疗糖尿病性下肢溃疡的疗效观察
Curative effect of intermittent irrigation with insulin solution combined with closed negative pressure drainage on lower limb diabetic chronic ulcer

Abstract
Objective:To investigate the curative effect of intermittent irrigation with insulin solution combined with closed negative pressure drainage on lower limb diabetic chronic ulcer.Methods:A total of 100 diabetic patients with chronic ulcer wounds of lower limbs in our hospital from January 2019 to January 2022 were selected.They were divided into two groups according to the randomized numerical table method:the control group(n=50)was treated with closed negative-pressure continuous drainage,and the study group(n=50)was treated with intermittent irrigation combined with insulin solution on the overall basis of the control group.The treatment effect,sarcomere coverage,sarcomere thickness,bacterial clearance,inflammatory factor level,vascular endothelial growth factor level,and the evaluation of the quality of life were compared between the two groups after 2 weeks.Results:After 2 weeks of treatment,the total effective rate in the study group(92%)was higher than that in the control group(74%),with statistical significance(P<0.05).The difference in bacterial clearance rate was statistically significant[(82.3±7.7)%vs(61.6±8.2)%,P<0.05].Compared with the control group,the granulation tissue in the study group grew faster[(61.5±7.6)%vs(30.5±7.9)%],and the granulation tissue was fuller[(3.52±0.29)mm vs(2.48±0.21)mm].The differences between the two groups were statistically significant(P<0.05).Before the therapy,the inflammatory indicators(IL-6,PCT,CRP)and VEGF levels were compared between the two groups,and the difference was not statistically significant(P>0.05).After 2 weeks of therapy,inflammatory indicators were lower in the study group compared with the control group { IL-6[(16.5±2.6)pg·mL-1 vs(23.5±4.2)pg·mL-1],PCT[(0.21±0.10)ng·mL-1 vs(0.5±0.19)ng·mL-1],CRP[(6.3±1.4)mg·mL-1 vs(12.6±2.2)mg·mL-1]},with statistical significance(P<0.05).After 2 weeks of treatment,the VEGF levels in the sdtudy group were higher[(585.6±4.8)pg·mL-1 vs(464.5±5.5)pg·mL-1],with statistical significance(P<0.05).Before treatment,the scores of patients'quality of life were compared between the two groups,with no statistical significance(P>0.05);after 2 weeks of treatment,the quality of life scores of the study group and the control group were(85.3±5.1)and(72.5±3.7)respectively,with statistical significance(P<0.05).Conclusion:The clinical effect of insulin solution intermittent irrigation combined with closed negative pressure drainage on the lower limb diabetes ulcer wound is relatively significant,which can promote the growth of granulation tissue and the overall secretion of growth factors,reduce the inflammatory reaction of the wound,accelerate ulcer healing,and effectively improve the quality of life of patients.关键词
