Abstract
Objective:To explore the influence of combination of fiberoptic bronchoscopy sputum aspiration and lavage combined with mechanical expectoration on pulmonary function and inflammatory factors in patients with pulmonary infection after severe cerebral hemorrhage surgery.Method:A total of 60 patients with pulmonary infection after severe cerebral hemorrhage surgery treated in the First People's Hospital of Longnan City from September 2021 to September 2023 were selected as the study objects and randomly divided into mechanical group and fiberoptic bronchoscopy group,with 30 cases in each group.Both groups were given basic treatment,the mechanical group was treated with vibration expectoration machine for mechanical expectoration,and the fiberoptic bronchoscopy group was treated with fiberoptic bronchoscopy sputum aspiration and lavage combined with mechanical expectoration.The curative effect of chest X-ray plain film was compared between the two groups.The pulmonary infection[clinical pulmonary infection score(CPIS)]of patients were evaluated,and the arterial blood gas indexes[arterial partial pressure of oxygen(PaO2),oxygenation index,arterial partial pressure of carbon dioxide(PaCO2),percutaneous arterial oxygen saturation(SpO2)],pulmonary function[peak airway pressure(Ppeak),plateau pressure(Pplat),airway resistance(Raw),driving pressure(DP),dynamic lung compliance(Cdyn)]and and inflammatory factors[procalcitonin(PCT),interleukin-6(IL-6),C reactive protein(CRP)]of patients were measured.Result:The total effective rate of fiberoptic bronchoscopy group was significantly higher than that of mechanical group,the difference was statistically significant(P<0.05).CPIS,PaCO2 and the levels of serum CRP,IL-6 and PCT in both groups after treatment were significantly lower than those before treatment,and those in the fiberoptic bronchoscopy group were significantly lower than those in the mechanical group,the differences were statistically significant(P<0.05).PaO2,oxygenation index and SpO2 in both groups after treatment were significantly higher than those before treatment,and those in the fiberoptic bronchoscopy group were significantly higher than those in the the mechanical group,the differences were statistically significant(P<0.05).Ppeak,Pplat,Raw and DP in fiberoptic bronchoscopy group after 3 days and 1 week of treatment were significantly lower than those in mechanical group,and Cdyn were significantly higher than those in mechanical group,the differences were statistically significant(P<0.05).Conclusion:The combined treatment of fiberoptic bronchoscopy sputum aspiration and lavage and mechanical expectoration for patients with pulmonary infection after severe cerebral hemorrhage surgery can achieve ideal curative effect,and the patients'pulmonary infection and pulmonary ventilation function can be effectively improved.关键词
