Abstract
Objective:To investigate the pathogenic distribution characteristics and high-risk factors of pulmonary infection after laparoscopic radical gastrectomy for gastric cancer.Method:Clinical data of 140 patients with gastric cancer admitted to Jiujiang Chaisang District People's Hospital from March 2020 to March 2023 were retrospectively analyzed.All patients underwent laparoscopic radical gastrectomy for gastric cancer,and they were divided into pulmonary infection group and non-pulmonary infection group according to whether pulmonary infection occurred during hospitalization.Baseline data such as age,sex,drinking history,smoking history,combined hypertension,combined diabetes,combined malnutrition,operation time,intraoperative blood loss,catheterization time,and tumor location were collected in the two groups.Univariate analysis was performed first,and logistic regression analysis was carried out later to obtain the high-risk factors affecting pulmonary infection after laparoscopic radical gastrectomy.The distribution characteristics of pathogenic bacteria in patients with pulmonary infection were analyzed.Result:Postoperative pulmonary infection occurred in 15 of 140 patients,the incidence was 10.71%(15/140).A total of 18 pathogenic bacteria were detected in 15 cases of pulmonary infection,of which 11 strains were Gram-negative(61.11%),6 strains were Pseudomonas aeruginosa(33.33%),3 strains were Klebsiella pneumoniae(16.67%)and 2 strains were Escherichia coli(11.11%);there were 7 strains of Gram-positive bacteria(38.89%),4 strains of Staphylococcus aureus(22.22%),2 strains of Staphylococcus hemolyticus(11.11%)and 1 strain of Staphylococcus epidermidis(5.56%).There were statistically significant differences in age,smoking history,combined diabetes,combined malnutrition and operation time between the two groups(P<0.05).logistic regression analysis showed that age≥60 years old,history of smoking,diabetes,malnutrition,and operation time≥3 h were the high-risk factors for pulmonary infection after laparoscopic radical gastrectomy(OR>1,P<0.05).Conclusion:The risk of pulmonary infection after laparoscopic radical gastrectomy for gastric cancer is high,which is closely related to age,smoking history,combined diabetes,combined malnutrition,and operation time.In addition,the pulmonary infection is mainly Gram-negative bacteria,and targeted antibacterial treatment is needed to accelerate the regression of infection.关键词
