脂蛋白(a)预测下肢动脉硬化闭塞症患者支架置入术后再狭窄的价值
The Value of Lipoprotein(a)in Predicting Restenosis after Stenting in Patients with Lower Extremity Arteriosclerotic Obliterans

Abstract
Objective:To investigate the value of lipoprotein(a)analysis in predicting restenosis after stenting in patients with lower extremity arteriosclerosis obliterans.Method:A total of 120 patients receiving stenting for lower extremity arteriosclerosis obliterans in the First People's Hospital of Lianyungang from March 2021 to January 2023 were retrospectively selected as the study objects,and were divided into a restenosis group of 50 cases and a non-restenosis group of 70 cases according to their restenosis status after stenting 12 months of follow-up.The general data,preoperative examination results and operation conditions of the two groups were compared,and the levels of lipoprotein(a)were compared at 3,6 and 12 months after surgery.The value of lipoprotein(a)in predicting restenosis after stenting was observed by drawing the receiver operator characteristic(ROC)curve.Result:Compared with the non-restenosis group,the restenosis group had higher hypertension,C reactive protein,stent length,stent number,lesion length,main iliac artery calcification,lipoprotein(a)level,and lower ankle-brachial index and serum albumin level(P<0.05).There were no significant differences in age,gender,diabetes,body mass index(BMI),patency of subknee outflow channels,and preoperative drug use between the two groups(P>0.05).The levels of lipoprotein(a)in non-restenosis group were lower than those in restenosis group at 3,6 and 12 months after operation(P<0.05).The area under curve(AUC)for restenosis prediction of lipoprotein(a)at 3,6 and 12 months after stent implantation was 0.744[95%CI(0.604,0.885)],0.731[95%CI(0.595,0.866)]and 0.704[95%CI(0.579,0.829)],respectively,the sensitivity was 60.05%,61.91%,63.32%,the specificity was 84.42%,78.94%,74.57%,respectively.Conclusion:Lipoprotein(a)has high sensitivity and specificity in predicting restenosis after stenting in patients with lower extremity arteriosclerosis obliterans,and has high predictive value and certain clinical use value.关键词
