Abstract
Objective:To study the expression level of serum cell adhesion molecule in multiple myeloma patients and its role and value in prognosis assessment.Method:Seventy-six patients with multiple myeloma from Ji'an Central People's Hospital from January 2020 to October 2022 were selected as the observation group,and 70 healthy volunteers who underwent physical examination in author's hospital during the same period were selected as the control group.General data of the two groups were collected,and serum levels of soluble vascular cell adhesion molecule-1(sVCAM-1),soluble intercellular adhesion molecule-1(sICAM-1)and mucin 1(MuC1)were detected and compared between the two groups.The observation group patients all received standardized chemotherapy.Based on the median progression-free survival(PFS)within one year of follow-up,all patients in the observation group were divided into a good prognosis group(n=38)and a poor prognosis group(n=38).The Cox proportional hazards model was employed to examine the determinants influencing patient outcomes,and Pearson's correlation analysis was utilized to investigate the associations between cell adhesion molecule levels and PFS,and the combined diagnostic efficacy of cell adhesion molecule for patient prognosis was evaluated using ROC curves.Result:The levels of sVCAM-1,sICAM-1 and MuC1 in observation group were higher than those in control group(P<0.05).The PFS of the poor prognosis group in the observation group was(4.58±1.27)months,and that of the good prognosis group was(10.26±2.18)months.Cox analysis showed that sVCAM-1,sICAM-1,and MuC1 were significant factors affecting patient prognosis and were negatively correlated with PFS(P<0.05).The ROC curve indicated that the combined diagnosis of cell adhesion molecule had a high diagnostic efficacy for patient prognosis,with an area under the curve reaching 0.952.Conclusion:Serum sVCAM-1,sICAM-1 and MuC1 are expressed at higher levels in patients with multiple myeloma,and combined detection has significant value in the assessment of prognosis.关键词
