Distribution of Septin9 Gene Methylation in Colorectal Cancer in Henan Province from 2020 to 2023
目的:调查肠癌Septin9基因甲基化在河南省的年龄、地域分布及临床特征,构建早筛风险模型,为临床肠癌早筛提供建议.方法:通过对2020~2023年间的20 707例肠癌Septin9基因甲基化检测数据进行回顾性分析,使用卡方检验和非线性算法进行模型模拟.结果:在20 707例体检样本中,整体肠癌Septin9基因甲基化检测阳性率为2.66%.样本按年龄分组从10岁到90岁,阳性峰值为50~60岁(6 087例,占比29.4%),80~90岁之间的风险率最高(5.37,P<0.001);不同季节的检测数量不同,冬季最高,但季节之间阳性率无显著差异(P>0.05);郑州、许昌、开封等中心城市的甲基化检测阳性率高于边缘城市,且呈现了中部高于北部,北部高于西部的特殊状态.男性风险率为2.83,显著大于女性(阳性率为2.19,P<0.001).研究发现63.27、74.54、85.82岁的人群甲基化风险分别是20岁的1.5倍,2倍和3倍.结论:本研究构建了河南首个地区性年龄与基因甲基化关联的风险模型,可为临床肠癌早筛工作提供重要建议.
Objective:To characterize the age,regional distribution and clinical characteristics of colorectal cancer pa-tients with Septin9 gene methylation in Henan Province,construct a early-screening risk model,and provide suggestions for clinical early screening of colorectal cancer.Methods:Septin9 gene methylation data from 20,707 cases of colorectal cancer from 2020 to 2023 was retrospectively analyzed,and the model was simulated by chi-square test and nonlinear algorithm.Re-sults:The overall positive rate of Septin9 gene methylation in colorectal cancer was 2.66%in 20,707 physical examination samples.Test samples were separated by age group,the number of positive samples peak in the 50~60 age group(6,087 cases,29.4%),with the highest risk rate in the 80~90 age group(5.37,P<0.001).The number of tests varied between seasons with the highest in winter.However,there was no significant difference in positive rates between seasons(P>0.05).The positive rate of methylation detection in central cities such as Zhengzhou,Xuchang and Kaifeng was higher than that in remote cities,and that in the central region was higher than that in the north region,and that in the north region was higher than that in the west region.The positive rate of males was 2.83 which was significantly higher than that of females(2.19,P<0.001).It was found that the risk of methylation at the ages of 63.27,74.54 and 85.82 were larger than that at the age of 20 by 1.5 times,2 times and 3 times.Conclusion:This study established the first regional age-gene methyla-tion risk model for Henan Province,which provides important guidance for the clinical early screening of colorectal cancer.
