紧密型县域医共体背景下基层医务人员满意度现状及影响因素研究
Satisfaction and influencing factors of grassroots medical staff under the context of the compact medical alliance within counties

Abstract
Objective To analyze the satisfaction of grassroots medical staff under the context of the com-pact medical alliance within counties and to provide references for its development and improvement,and the pro-motion of medical service capacity of grassroots healthcare institutions.Methods A questionnaire survey was im-plemented using stratified sampling in grassroots healthcare institutions in Sanming City.T-tests,one-way ANO-VA,or Kruskal-Wallis rank-sum tests were used for between-group comparisons,and ordered logistic regression was used to analyze the influencing factors for overall satisfaction.Results After the implentation of compact medical alliance within counties,the overall satisfaction score of medical staff in grassroots healthcare institutions of Sanming City was 4.29±0.73,and 793(85.36％)were generally satisfied.Factors affecting the satisfaction of grassroots medical staff were satisfaction with training and learning provided by the institution(OR=2.09,95％CI=1.65～2.65),satisfaction with professional title promotion and appointment(OR=3.66,95％CI=2.85～4.70),satisfaction of the population with the grassroots healthcare institutions(OR=2.41,95％CI=1.42～4.10),recognition of basic public health service(ORbetter=2.02,95％CI=1.04～3.93;ORvery good=7.70,95％CI=3.63～16.35)and workload changes(OR=0.53,95％CI=0.31～0.91).Conclusions Under the context of the compact medical alliance within counties,the overall satisfaction of grassroots medical staff is high.It is suggested to increase the salary of grassroots medical staff,improve the performance appraisal and professional title promotion and appointment system,and strengthen training to improve the overall capacity of grassroots medi-cal staff,so as to provide more support for the sustainable development of the compact medical alliance within counties.关键词
