紧密型医联体背景下医防融合慢病管理体系的探讨
Exploration of an Integrated Chronic Disease Management System Combining Medical Treatment and Prevention within the Context of a Compact Medical Consortium

Abstract
Integration of medical care and prevention refers to breaking down institutional barriers between the public health service system and the medical service system to achieve the goals of improving the utilization rate of health resources and the health status of the population by relying on information technology and integrating pub-lic health resources with medical resources.However,there are still few studies on integration of medical care and prevention in China and there are differences in the implementation models of the integration among various re-gions,indicating the lack of corresponding standard specifications.To improve and optimize the integrated medical care and chronic disease management system,the Second Nanning People's Hospital took the opportunity of the Guangxi Science and Technology Major Programs and built an integrated medical care and chronic disease manage-ment system for six common and frequently-occurring diseases through a series of measures,including overall plan-ning,coordinated layout,standardized processes,and information construction.This study introduces the experi-ence and practice in building this system to provide references for other hospitals to carry out the same work.关键词
