针刺联合祛风通络汤治疗恢复期周围性面瘫患者的效果观察OA
Observation on the Effect of Acupuncture Combined with Qufeng Tongluo Decoction in the Treatment of Convalescent Facial Nerve Paralysis Patients
目的:探讨针刺联合祛风通络汤治疗恢复期周围性面瘫(FNP)患者的效果.方法:选取 2019年 8月—2023年 8 月南丰县中医院收治的 84例FNP患者为研究对象,根据随机数字表法将其分为对照组和研究组,各 42例.对照组采用针刺治疗,研究组在对照组基础上联合祛风通络汤治疗.比较两组治疗效果、面神经功能、面肌肌电图指标、面部功能障碍程度、中医症状积分.结果:研究组治疗总有效率为 95.24%,高于对照组的 80.95%,差异有统计学意义(P＜0.05).治疗后,两组Portmann、多伦多面神经分级系统量表评分均高于治疗前,且研究组均高于对照组,差异均有统计学意义(P＜0.05).治疗后,两组眼轮匝肌、口轮匝肌动作电位的潜伏期均短于治疗前,M波波幅均大于治疗前,且研究组眼轮匝肌、口轮匝肌动作电位的潜伏期均短于对照组,M波波幅均大于对照组,差异均有统计学意义(P＜0.05);治疗后,研究组面部残疾指数量表各个部分评分均高于对照组,差异均有统计学意义(P＜0.05);治疗后,两组各项中医症状积分均低于治疗前,且研究组口眼歪斜、眼睑闭合不全、额纹消失积分均低于对照组,差异均有统计学意义(P＜0.05).结论:针刺联合祛风通络汤治疗恢复期FNP患者,有助于改善疗效,加快面部神经和肌肉功能的恢复,改善面部残疾程度和症状.
Objective:To investigate the effect of acupuncture combined with Qufeng Tongluo Decoction on convalescent facial nerve paralysis(FNP).Methods:A total of 84 patients with FNP admitted to Nanfeng County Hospital of Traditional Chinese Medicine from August 2019 to August 2023 were selected as the study objects,and were divided into a control group and a study group according to random number table method,with 42 cases in each group.The control group was treated with acupuncture,and the study group was treated with Qufeng Tongluo decoction on the basis of the control group.The therapeutic effect,facial nerve function,facial electromyography index,degree of facial dysfunction and TCM symptom score were compared between the two groups.Results:The total effective rate of 95.24%in the study group was higher than 80.95%in the control group,the difference was statistically significant(P＜0.05).After treatment,the scores of Portmann and Toronto Facial Nerve Grading System Scale in both groups were higher than those before treatment,and the study group was higher than that in the control group,the differences were statistically significant(P＜0.05).After treatment,the incubation period of the orbicularis oculi muscle,orbicular muscle action potentials of two groups were shorter than before the treatment,M wave amplitude were greater than those before the treatment,and the incubation period of the study group were shorter than the control group,M wave amplitude of the study group were greater than those in the control group,the differences had statistical significance(P＜0.05);after treatment,the scores of all parts of Facial Disability Index Scale in the study group were higher than the control group,with statistical significance(P＜0.05).After treatment,the scores of various TCM symptoms in both groups were lower than those before treatment,and the scores of mouth and eye skew,incomplete eyelid closure and frontal striae disappearance in the study group were lower than those in the control group,with statistical significance(P＜0.05).Conclusion:Acupuncture combined with Qufeng Tongluo Decoction can improve the curative effect,accelerate the recovery of facial nerve and muscle function,and improve the degree of facial disability and symptoms in patients with FNP in convalescence.
