摘要

Abstract

Objective:To explore the application effect of Community Standardized Metabolic Disease Management Platform(MMC)in the management of early elderly diabetic nephropathy patients.Methods:132 cases of early se-nile diabetic nephropathy in our center from January 2022 to December 2023 were randomly divided into control group and research group,with 66 cases in each group.The control group received routine management,while the study group utilized MMC.Blood glucose control,renal function indicators,and complication rates were assessed before and after a 6-month intervention.Results:Post-intervention,the study group showed significantly lower levels of FPG、2hPG and HbA1c compared to the control group(P<0.05).The study group also demonstrated higher eGFR and lower UACR(P<0.05).Moreover,the complication rate was significantly reduced in the study group(P<0.05).Conclusion:The Community Standardized Metabolic Disease Management Platform(MMC)can effectively improve blood glucose control and renal function,and reduce the incidence of complications in early elder-ly diabetic nephropathy patients,which is worthy of clinical promotion and application.
