目的 研究浓缩生长因子(concentrated growth factor,CGF)联合牙周加速成骨正畸术(periodontally acceler-ated osteogenic orthodontics,PAOO)对薄龈型患者下前牙牙周软硬组织改建的效果.方法 选取2023年1月至2024年4月于合肥市口腔医院正畸科就诊并转诊至牙周科需行下前牙区PAOO的薄龈型患者20例(下前牙118颗),采用随机数字表法分为研究组(10例,下前牙58颗)和对照组(10例,下前牙60颗).研究组在PAOO术中植入质量比1:1的CGF凝胶与Bio-Oss骨粉,覆盖CGF膜+胶原膜;对照组术中仅植入Bio-Oss骨粉,覆盖胶原膜.比较2组术前和术后6个月探诊深度(probing depth,PD)、出血指数(bleeding index,BI)、牙龈退缩深度(recession depth,RD)、角化龈宽度、牙龈厚度、唇侧根中部骨厚度的变化.结果 术前和术后6个月,研究组与对照组的PD、BI和RD比较,差异均无统计学意义(均P>0.05).术后6个月,研究组和对照组唇侧根中部骨厚度分别较术前增加(1.14±0.24)、(1.03±0.26)mm,牙龈厚度分别较术前增加(0.35±0.15)、(0.17±0.15)mm;2组唇侧根中部骨厚度、牙龈厚度增加量比较,差异有统计学意义(t值分别为2.340、6.388,均P<0.05).2组角化龈宽度较术前也均有增加,但其增加量比较,差异无统计学意义(P>0.05).术后6个月,研究组牙龈厚度>1 mm的下前牙38颗(占65.5%),对照组牙龈厚度>1 mm的下前牙17颗(占28.3%),二者差异有统计学意义(x2=16.386,P<0.001).结论 CGF应用于PAOO术可改善薄龈型患者的牙周表型,提升术后下前牙牙周软硬组织改建的效果.

Objective To study the effect of concentrated growth factor(CGF)combined with periodontally accelerated osteogenic orthodontics(PAOO)on periodontal soft and hard tissue reconstruction in the lower anterior teeth of patients of thin gingival phenotype.Methods Twenty patients of thin gingival phenotype(118 lower anterior teeth)were select-ed,who attended the orthodontic department of Hefei Stomatological Hospital and were referred to the periodontal depart-ment for PAOO in the lower anterior region from January 2023 to April 2024,and they were divided into a study group(10 cases,58 lower anterior teeth)and a control group(10 cases,60 lower anterior teeth)by using a randomised numeri-cal table method.The study group was implanted with CGF gel and Bio-Oss bone powder at a mass ratio of 1:1,covered with a CGF membrane+collagen membrane during the PAOO procedure,and the control group was implanted with Bio-Oss bone powder only,covered with a collagen membrane during the procedure.Changes in probing depth(PD),bleeding index(BI),gingival recession depth(RD),keratinised gingival width,gingival thickness,and labial mid-root bone thickness were compared between the 2 groups preoperatively and 6 months postoperatively.Results Preoperatively and at 6 months postoperatively,there was no statistically significant difference in PD,BI or RD between the study group and the control group(all P>0.05).At 6 months postoperatively,the bone thickness in the labial side of the mid-root increased by(1.14±0.24)mm and(1.03±0.26)mm,and the gingival thickness increased by(0.35±0.15)mm and(0.17±0.15)mm in the study group and the control group,respectively,compared with the preoperative period;the difference in the increase in the thickness of the bone in the labial side of the mid-root and the gingival thickness in the 2 groups was statistically significant(t val-ues being 2.340,6.388,both P<0.05).The keratinised gingival width also increased in both groups compared to the pre-operative period,but the difference was not statistically significant when comparing the amount of increase(P>0.05).At 6 months postoperatively,there were 38 lower anterior teeth with gingival thickness>1 mm(65.5%)in the study group and 17 lower anterior teeth with gingival thickness>1 mm(28.3%)in the control group,and the difference between the two was statistically significant(x2=16.386,P<0.001).Conclusion CGF applied to the PAOO procedure improves the periodontal phenotype in patients of thin gingival phenotype and enhances the postoperative periodontal soft and hard tissue reconstruction of the lower anterior teeth.
