目的 应用定量光导荧光(quantitative light-induced fluorescence,QLF)技术及扫描电镜观察并评价酪蛋白磷酸肽-无定形磷酸钙(casein phosphopeptides-amorphous calcium phosphate,CPP-ACP)联合多乐氟对人工制备釉质早期龋的再矿化作用.方法 收集2020年10月至2021年8月于右江民族医学院附属医院口腔科就诊患者因正畸治疗需拔除的前磨牙34颗,制备釉质早期龋模型.随机选取2颗离体牙用于观察脱矿处理后的釉质表面形态,记为空白对照牙;将剩余的32颗离体牙行再矿化处理,并随机分为4组,每组8颗,依据处理方法记为去水离子水组、CPP-ACP组、多乐氟组、联合组(CPP-ACP+多乐氟).应用QLF图片测量比较各组再矿化处理前及再矿化处理21 d后的荧光损失量百分比(△F)、脱矿面积(Area)及总荧光损失量(△Q),并在扫描电镜下观察各组再矿化处理21 d后釉质表面形态.结果 再矿化处理前,4组△F、Area、△Q比较,差异均无统计学意义(均P>0.05).再矿化处理21 d后,与再矿化处理前比较,去离子水组、CPP-ACP组、多乐氟组、联合组△F的绝对值、Ar-ea、△Q 的绝对值均减小,差异均有统计学意义(P<0.05);且4组△F、Area、△Q比较,差异均有统计学意义(均P<0.05);其中,联合组△F的绝对值、Area、△Q的绝对值均小于其他3组,差异均有统计学意义(均P<0.05).扫描电镜下观察发现,4组相较于空白对照牙均有矿化表现,其中去离子水组改变较小,CPP-ACP组、多乐氟组、联合组均出现了明显的再矿化现象,且联合组再矿化效果更佳.结论 相较于单独使用CPP-ACP和多乐氟,二者联合作用于釉质早期龋模型,会有更佳的再矿化效果.

Objective To evaluate the remineralization effect of casein phosphopeptide-amorphous calcium phosphate(CPP-ACP)combined with Duraphat on artificially induced early enamel caries using quantitative light-induced fluores-cence(QLF)technology and scanning electron microscopy(SEM).Methods Thirty-four extracted premolars from pa-tients who sought treatment at the Department of Stomatology,Youjiang Medical University for Nationalities Affiliated Hospital,from October 2020 to August 2021 were used to prepare early enamel caries models.Two extracted teeth were randomly selected for observation of the surface morphology of the enamel after demineralization,which was recorded as the blank control tooth.The remaining 32 extracted teeth were subjected to remineralization treatment,and were random-ly divided into four groups of eight teeth each;according to the treatment method,they were recorded as deionized water group,CPP-ACP group,Duraphat group,and combined group(CPP-ACP+Duraphat).The QLF images were used to compare the fluorescence loss percentage(△F),deminer-alized area(Area),and lesion volume(△Q)before and af-ter 21 days of remineralization treatment in each group,and the surface morphology of the enamel at 21 days after remineralization treatment was observed under SEM.Results Before remineralization treatment,there was no signifi-cant difference in △F,Area,or △Q among the four groups(P>0.05).After 21 days of remineralization treatment,com-pared with the pretreatment,the absolute value of △F,△Q,and Area of the CPP-ACP group,deionized water group,Du-raphat group,and combined group decreased,and the differences were statistically significant(P<0.05).Compared with the other three groups,the absolute value of △F,△Q,and Area of the combined group were the smallest,and the differ-ences were statistically significant(P<0.05).Under SEM observation,it was found that all four groups showed reminer-alization compared with the blank control teeth,with the changes in the deionized water group being smaller,and the CPP-ACP group,the Duraphat group,and the combined group showing obvious remineralization.The combined group showed better remineralization effect.Conclusion Compared with using CPP-ACP or Duraphat alone,using them to-gether for early enamel caries model will have better remineralization effects.
