目的 探讨抗阻运动对稳定性冠心病患者的运动耐量及血管内皮调节因子的影响.方法 采用前瞻性队列研究的方法,纳入2020年10月至2022年10月包头医学院第二附属医院稳定性冠心病患者 100 例,按入组顺序随机分为对照组及抗阻运动组,最终 77 例纳入研究,其中对照组 36 例,抗阻运动组 41 例.对照组给予标准化药物治疗;抗阻运动组在标准化药物治疗基础上,按照运动处方进行低强度抗阻运动 12 周.采用 6 min步行距离评估患者运动耐量;采用ELISA法检测外周血血管内皮生长因子(VEGF)、一氧化氮(NO)、内皮素-1 的水平.比较两组患者入组时、12 周后 6 min步行距离及VEGF、NO、内皮素-1 水平.随访 24 周比较两组患者严重心血管不良事件发生情况.结果 (1)与运动前比较,抗阻运动组运动后 6 min步行距离增加,静息收缩压下降,静息舒张压下降(t=7.538、2.290、2.639,P<0.05);抗阻运动组运动后VEGF、NO升高,内皮素-1 下降(t=5.509、6.089、5.824,P<0.05).(2)与对照组比较,12 周后抗阻运动组 6 min步行距离增加、VEGF及NO水平升高、内皮素-1 水平下降(t=3.504、3.559、3.502、3.645,P<0.05).(3)抗阻运动组运动前后6 min步行距离差值与运动前后VEGF差值、NO差值成正相关(r=0.385、0.485,P<0.05);与内皮素-1 差值成负相关(r=-0.510,P<0.01).(4)随访 24 周,两组冠心病患者严重心血管不良事件发生率(8.33%比 9.76%)比较差异无统计学意义(P>0.05).结论 低强度抗阻运动可以升高VEGF及NO的水平、降低内皮素-1的水平,提高稳定性冠心病患者的运动耐量.

Objective To investigate the effects of resistance exercise on exercise tolerance and vascular endothelial regulatory factors in patients with stable coronary artery disease(CHD).Methods A prospective cohort study was conducted,including 100 patients with stable CHD from October 2020 to October 2022 at the Second Affiliated Hospital of Baotou Medical College.Patients were randomly divided into a control group and a resistance exercise group,with a final study population of 77 patients,consisting of 36 in the control group and 41 in the resistance exercise group.The control group received standardized drug treatment,while the resistance exercise group performed low-intensity resistance exercise for 12 weeks according to an exercise prescription in addition to standardized drug treatment.The 6-minute walk distance was used to assess exercise tolerance,and ELISA was used to measure the levels of vascular endothelial growth factor(VEGF),nitric oxide(NO),and endothelin-1 in peripheral blood.The 6-minute walking distance,levels of VEGF,NO,and endothelin-1 were compared before and after 12 weeks of treatment in both groups.The incidence of serious cardiovascular adverse events was compared between the two groups after a 24-week follow-up.Results(1)Compared with before exercise,the 6-minute walking distance increased,while the resting systolic blood pressure and the resting diastolic blood pressure decreased in the resistance exercise group(t=7.538,2.290,2.639;P<0.05);VEGF and NO increased,while endothelin-1 decreased in the resistance exercise group(t=5.509,6.089,5.824;P<0.05).(2)Compared with the control group,the resistance exercise group had increased 6-minute walking distance,increased VEGF and NO levels,and decreased endothelin-1 level(t=3.504,3.559,3.502,3.645;P<0.05)after 12 weeks.(3)The difference in 6-minute walking distance before and after exercise in the resistance exercise group was positively correlated with the difference in VEGF and NO(r=0.385,0.485;P<0.05),and negatively correlated with the difference in endothelin-1(r=-0.510,P<0.01).(4)After a 24-week follow-up,there was no statistically significant difference in the incidence of serious cardiovascular adverse events between the two groups of CHD patients(8.33%vs.9.76%,P>0.05).Conclusion Low-intensity resistance exercise can increase the levels of VEGF and NO,decrease the level of endothelin-1,and improve the exercise tolerance of patients with stable CHD.
