Abstract

Objective To investigate the effect of vacuum sealing drainage(VSD)combined with insulin,gentamicin and vitamin B12 irrigation on levels of Bcl-2 and Bax in the wound surface and tissue of diabetes foot.Methods 55 patients with diabetic foot were selected as study subjects,and they were divided into control group(20 cases,applying VSD to treat diabetic foot wounds),and the experimental group(35 cases,applying VSD combined with insulin,gentamicin,and vitamin B12 to flush the wound surface of diabetes foot)according to different treatment methods.The wound area and the levels of Bcl-2 and Bax in the wound tissue of two groups were compared 7 days and 14 days after treatment.Results There was no significant difference in the wound surface of two groups 7 days after treatment(P>0.05),and there was no significant difference in the expression level of Bax between experimental group and control group(P>0.05).The level of Bcl-2 in experimental group was significantly higher than that in control group(P<0.05).After 14 days of treatment,the wound area in experimental group was significantly smaller than that in control group(P<0.05),and there was no significant difference in Bax levels compared to control group(P>0.05).Bcl-2 levels were significantly higher than those in control group(P<0.05),and the Bcl-2/Bax ratio was significantly higher than that in control group(P<0.05).Conclusion The mechanism of action of VSD combined with insulin,gentamicin,and vitamin B12 rinse for the treatment of diabetic foot wounds involves up-regulating the Bcl-2 levels in the wound tissue by activating the Bcl-2-mediated anti-apoptotic pathway and increasing the Bcl-2/Bax ratio.This approach aims to inhibit local apoptosis and promote wound healing.
