双胎妊娠合并妊娠期高血压疾病的临床特点及妊娠结局
Clinical characteristics and pregnancy outcomes of twin pregnancy complicated with hypertensive disorders of pregnancy

Abstract
Objective To examine the clinical characteristics and pregnancy outcomes associated with hyperten-sive disorders of pregnancy(HDP)in twin pregnancies,providing insights to improve the management of twin pregnancies complicated by HDP.Methods A retrospective analysis was conducted on 3 564 pregnant women who delivered at the Fetal Medicine Department of Guangxi Zhuang Autonomous Region Maternal and Child Health Hospital from January 2020 to December 2022.Among these,2 869 had singleton pregnancies,and 695 had twin pregnancies.HDP cases were divid-ed into two groups:the twin HDP group(120 cases)and the singleton HDP group(218 cases).The clinical characteris-tics and pregnancy outcomes of twin pregnancies complicated by HDP were analyzed and compared.Results The overall incidence of HDP was 9.5％(338/3 564),with a significantly higher rate in twin pregnancies(17.2％)compared to singleton pregnancies(8.1％)(P＜0.05).The incidence of preeclampsia-eclampsia was significantly higher in the twin HDP group(65.0％)than in the singleton HDP group(37.6％)(P＜0.05).The twin HDP group showed in-creased rates of oligohydramnios,hypoproteinemia,and anemia(20.0％,6.7％,and 68.3％,respectively)compared to the singleton HDP group(6.9％,0.9％,and 52.3％,respectively)(P＜0.05).Rates of cesarean section,preterm birth,postpartum hemorrhage,and low birth weight infants were significantly higher in the twin HDP group(92.5％,65.8％,18.3％,and 62.9％,respectively)compared to the singleton HDP group(48.2％,24.8％,3.7％,and 31.2％,respectively)(P＜0.05).Additionally,the neonatal birth weight in the twin HDP group(2 225.33±522.73 g)was significantly lower than that in the singleton HDP group(2 735.32±787.63 g)(P＜0.05).Conclusion The incidence of HDP is significantly higher in twin pregnancies than in singleton pregnancies.Twin pregnancies complicated by HDP are associated with an increased risk of oligohydramnios,hypoproteinemia,anemia,and adverse pregnancy out-comes,such as cesarean delivery,preterm birth,postpartum hemorrhage,and low birth weight infants.Enhanced prenatal care and perinatal management are essential to improving outcomes in twin pregnancies with HDP.关键词
