Abstract
Chronic diseases seriously threaten the improvement of people's health,and the academic community is actively exploring the use of intelligent methods for health intervention and management of chronic diseases.Based on the concept of"three early"active health management,this paper introduces an AI assistant into the 5G+community chronic disease health management system.It has developed core algorithm modules such as"chronic disease patient profiling""chronic disease management quality control model""health assessment module"and"chronic disease assisted decision-making model"through the use of big data and digital technology research and development,and applies them in practice.The practical results confirm that the 5G+AI"three early"chronic disease active health management model can effectively monitor,govern,and apply data for the"two chronic diseases"population,providing a scientific basis for the promotion and application of the medical prevention integration service model in primary medical institutions.关键词
