Establishment and Validation of a Prognosis Prediction Model for Resect-able Lung Squamous Cell Carcinoma Patients Based on Methylation and Clinical Features

Abstract
Objective:The studies on the relationship between methylation alterations and prognosis of lung squamous cell carcinoma have been limited.This study aims to clarify the methylation features of lung squamous cell carcinoma(LUSC),and establish a model for prognosis prediction with both methylation and clinical features,and validate the model.Methods:Genome-wide methylation data of 245 patients with stage Ⅰ~Ⅲ resectable LUSC were downloaded from The Cancer Genome Atlas database and assigned to a training cohort.The tissue samples of 106 patients from our hospital with stage Ⅰ~ⅢLUSC in the last 5 years were retrospectively collected.Whole genome bisulfite sequencing was performed to establish a vali-dation cohort.R software was used for all analyses and figure plotting.Results:Patients with stage Ⅰ~Ⅲ resectable LUSC showed a large number of hypermethylated or hypomethylated genes.Enrichment analysis revealed multiple tumor-related functions or pathways.Univariate Cox regression analysis found 881 genes which were significantly associated with prognosis(P<0.05).Multivariate Cox regression analysis further revealed that 5 methylated genes,tumor stage and location were independent risk factors of prognosis(P<0.05).These factors were used to establish a prognosis prediction model.Subsequent internal and external validation showed satisfactory 1~5 year prediction performance,and the maximal AUC was achieved at the fifth year,reaching 0.82(95%CI:0.75~0.89)and 0.75(95%CI:0.68~0.82)in internal and external validation,respectively.Conclusion:This study has suc-cessfully characterized the methylation signatures in LUSC,and established a prognosis prediction model combining methyla-tion with clinical features.The model was successfully validated and may facilitate the prognosis prediction and therapeutic strategy establishment of LUSC patients.关键词
