目的:评估孔径形态控制器(aperture shape controller,ASC)算法在脑转移、肺癌和肝癌立体定向放射治疗(stereotactic body radiation therapy,SBRT)中的应用.方法:选取脑转移、肺癌和肝癌患者各20例做回顾性分析,计划系统为Eclipse 15.6,处方剂量分别为50 Gy/10F、40 Gy/5F和40 Gy/5F.采用容积旋转调强放疗(volumetric mod-ulated arc therapy,VMAT),分别选取 ASC 算法中的 very low(VMATVL)、moderate(VMATM)和 very high(VMATVH)参数设置3组计划.采用单因素方差分析对3组计划下的靶区D90、D2、适形度指数、梯度指数和危及器官的剂量学参数以及机器跳数(monitor unit,MU)和调制复杂度分数(modulation complexity score,MCS)进行比较.结果:3组计划靶区的差异均无统计学意义(P>0.05).脑转移计划中,VMATVL组脑组织的V5优于VMATVH组,VMATVH组的验证通过率高于VMATVL和VMATM组,差异均有统计学意义(P<0.05);肺癌计划中,VMATM组肺组织D1000cc和D1500cc优于VMATVH组,VMATVH组的MCS高于VMATVL和VMATM组,VMATVH组的验证通过率高于VMATVL组,VMATVL组的MU高于VMATM和VMATVH组,差异均有统计学意义(P<0.05);肝癌计划中,VMATVL组的食管Dmax优于VMATM组,VMATVH组的 MCS 高于 VMATVL和 VMATM 组(P<0.05),VMATVH组的验证通过率高于 VMATM 组(P<0.05),VMATVL组的MU高于VMATVH组,差异有统计学意义(P<0.05).结论:根据不同的临床需求,如优先考虑危及器官的保护,对于体积较小的脑转移、肺癌和肝癌的SBRT建议ASC设置选取very low;如优先考虑照射效率和验证通过率,对于体积较小的脑转移、肺癌和肝癌的SBRT建议ASC设置选取very high.

Objective:To evaluate the application of algo-rithm using aperture shape controller(ASC)in stereotactic body radiation therapy(SBRT)for brain metastasis,lung cancer and liver cancer.Methods:A retrospective analysis was performed for the radiotherapy plans for 20 brain metastasis patients,lung cancer patients and liver cancer patients,re-spectively.The planning system was Eclipse 15.6,and the prescription dose was 50 Gy/10 F,40 Gy/5 F and 40 Gy/5 F.Volumetric modulated arc therapy(VMAT)plan was designed in three ASC parameters,including very low(VMATVL)group,moderate(VMATM)group and very high(VMATVH)group.Target D90,D2,conformance index,gradient index and dosimetric parameters of organs at risk,monitor unit(MU)and modulation complexity score(MCS)were analyzed and compared among the three groups.One-way analysis of variance(one-way ANOVA)was used for comparison among groups.Results:There were no significant differences among three groups in dosimetry parameters for target(P>0.05).In the brain metastasis plan,the V5 of brain tissue in the VMATVL group was better than in the VMATVH group,γ passing rate in the VMATVH group was higher than those in VMATVL and VMATM groups,the differences were statistically significant(P<0.05).In the lung cancer plan,the results of D1000cc and D1500cc to lung in the VMATM group were better than those in the VMATVH group;MCS in the VMATVH group was higher than those in VMATVL and VMATM groups;γ passing rate in the VMATVH group was higher than that in the VMATVL group;MU in the VMATVL group was higher than those in VMATM and VMATVH groups,the differences were statistically significant(P<0.05).In the liver cancer plan,the Dmax of esophagus in the VMATVL group was better than that in the VMATM group;the MCS in the VMATVH group was higher than those in VMATVL and VMATM groups(P<0.05);the y passing rate in the VMATVH group was higher than that in the VMATM group(P<0.05);the MU in the VMATVL group was higher than that in VMATVH group,the difference was statistically significant(P<0.05).Conclusion:According to different clinical needs,such as prioritizing the protection of organs at risk,the rec-ommended ASC setting for SBRT for small volume brain metastasis,lung cancer and liver cancer is very low.If delivery effi-ciency and γ passing rate are prioritized,the ASC setting is recommended to be very high for SBRT for small volume brain metastasis,lung cancer and liver cancer.
