住院患者严重过敏反应病例特点分析
Analysis of Characteristics of Inpatients with Anaphylaxis

Abstract
Objective To analyze the characteristics of inpatients with anaphylaxis to provide reference for clinical practice.Methods The clinical characteristics of inpatients with anaphylaxis in Linfen People's Hospital from 2015 to 2023 were retrospectively analyzed,including the general conditions,underlying disea-ses,cause of anaphylaxis,clinical manifestations,blood pressure,electrolytes,medication,remission time and prognosis.Results The age of patients with anaphylaxis ranged from 2 years old to 74 years old,with mean age at(50±19)years old.Male:female=1∶0.72.Drugs(19/31)were the main cause of anaphylaxis,with the most common being contrast medium(7/19),followed by antibiotics(6/19).Cutaneous manifesta-tions(83.9%)were the most common clinical manifestations of anaphylaxis,followed by circulatory system(80.6%),nervous system(67.7%),respiratory system(61.3%),and digestive system(38.7%).Most patients(87.1%)experienced a drop in blood pressure.Patients'baseline blood pressure was[systolic blood pressure(122.9±22.1)mmHg and diastolic blood pressure(72.8±16.0)mmHg],The blood pres-sure of patients with anaphylaxis significantly decreased to[systolic blood pressure(64.7±26.9)mmHg and diastolic blood pressure(37.3±16.3)mmHg],and the difference was statistically significant(P<0.01).Patients'blood potassium was(4.03±0.34)mmol/L at baseline and decreased to(3.56±0.45)mmol/L at onset.Blood glucose increased from(5.39±0.92)mmol/L in the basal state to(10.58±4.22)mmol/L at attack while the blood potassium decreased(P<0.01)and the blood glucose increased(P<0.05),the difference was statistically significant.The basic state of serum sodium,blood chloride and serum calcium had no obvious change compared to the onset.Glucocorticoids were the common treatment,followed by epineph-rine.Advanced age and underlying cardiopulmonary diseases may be risk factors for the increase of mortali-ty.Conclusion Drugs are the main cause of anaphylaxis,with contrast medium as the most common one.The clinical manifestation of skin system was mostly seen.When anaphylaxis occurred,most patients'blood pres-sure and blood potassium decreased significantly and blood glucose increased.Advanced age and underlying car-diopulmonary diseases may be risk factors for the increase of mortality.关键词
